ObjectiveTo assess the correlation between 18F-fluorodeoxyglucose positron emission tomography/computed tomography(18F-FDG PET/CT)metabolic parameters with the mutation status of the epidermal growth factor receptor(EGFR)in patients with advanced non-small cell lung cancer(NSCLC),and to investigate the effect of the EGFR mutation status on the evaluation of prognosis by metabolic parameters.Methods109 cases of advanced NSCLC patients who had undergone both 18F-FDG PET/CT and EGFR mutation status evaluation in the First Affiliated Hospital of Chongqing Medical University,between January2017 to December 2018,were retrospectively analyzed.The patients were divided into the EGFR mutant group and the EGFR wild-type group according to their EGFR mutation status.The metabolic parameters and clinical information were then compared between the two groups,and their relation with the patients’ EGFR mutation status was analyzed.The progression free survival(PFS)of these patients was followed up to analyze the influence of the EGFR mutation status on the prognosis assessment by 18F-FDG PET/CT.ResultMultivariate logistic analysis demonstrated that female,adenocarcinoma,and SUVmax≤9.58 were correlated with the EGFR mutation.The receiver operating characteristic(ROC)curves of PFS illustrated that total lesion glycolysis(TLG)and metabolic tumor volume(MTV)could be used to evaluate the prognosis of NSCL patients.According to Cox regression analysis results,patients with MTV>14.85 in the EGFR mutant group and patients with MTV>12.48 in EGFR wild-type group had a higher risk of disease progression.ConclusionThe EGFR mutation in advanced NSCLC patients is related to low FDG uptakes,and volumetric parameters such as MTV、TLG are of great significance in their prognosis predication.However,different evaluation criteria should be adopted in the evaluation of prognosis of patients with different types of EGFR mutations by FDG PET/CT parameters.
