Research On The Construction Of The Evaluation Index System Of The Elderly’s Health Service Access

PurposeTo define the connotation of "sense of access to medical and health services" based on the previous theoretical research on sense of acquisition;to construct an evaluation index system for the sense of access to medical and health services for the elderly in combination with Delphi expert advice;to carry out empirical research in sample areas,Use survey data to revise the evaluation index system of the elderly’s medical and health service access,and try to obtain the evaluation index system of the elderly’s medical and health service accessibility that is in line with the actual situation in my country and has certain reference significance.Methods(1)Literature research method: Perform literature search and related data query on Chinese and foreign databases such as CNKI,Wanfang,Weipu,Pubmed and the Internet,and summarize the relevant research on the connotation of sense of acquisition at home and abroad and the evaluation index system,and make a preliminary analysis The relevant indicators for the evaluation of the elderly’s medical and health service accessibility evaluation have laid a good foundation for the construction of the scale.(2)Delphi method: After two rounds of expert consultation Delphi method,according to the importance scores and specific opinions of experts,the first draft of the evaluation index system for elderly medical and health services was obtained.(3)On-site investigation method: conduct on-site investigation on the sense of access to medical and health services for the elderly in the sample areas of Chongqing.(4)Exploratory factor analysis: through the exploratory factor analysis of principal component analysis,the number of common factors is determined and the score of common factors is calculated to revise the first draft of the evaluation index system for elderly medical and health services.Results(1)According to the previous literature research,combined with the characteristics of the health needs of the elderly,a selection database of evaluation indicators for elderly medical and health services was obtained that included 8 first-level indicators and 31 second-level indicators.(2)The results of two rounds of expert consultation: the positive coefficients of the two rounds of expert consultation were 0.833 and 1,respectively,the expert authority coefficient Cr was 0.87,and the coordination coefficients of the primary and secondary indicators were 0.355 and0.238(P<0.01),respectively.The first draft of the evaluation index system for elderly medical and health services includes 8 first-level indicators and 30 second-level indicators.(3)Revision of the evaluation index system: A total of 547 questionnaires were issued in this survey,and 540 valid questionnaires were returned.The effective rate of the questionnaire was 98.7%.The cumulative variance contribution rate of the six common factors obtained by exploratory factor analysis was70.143%.The revised evaluation index system of elderly medical and health service accessibility includes 6 first-level indicators and 28second-level indicators.The total Cronbach α coefficient is 0.859,and the Cronbach α coefficients of the six first-level indicators are between0.793-0.980.The overall sense of access to medical and health services for the elderly in Chongqing is 2.85±0.35,and the overall sense of gain is weak;specifically,there are four categories: priority of medical treatment for the elderly,sense of access to rehabilitation care services,sense of access to traditional Chinese medicine services,and sense of access to health education services.The sense of dimensionality needs to be improved.ConclusionBased on the acquisition related research,this study using the literature methodology,the Delphi expert consultation method,methods of exploratory factor analysis,definition of the connotation of medical and health services the acquisition,set up and revised the elderly health service evaluation index system of the acquisition,has certain scientific nature and reliability,can evaluate the elderly health service provide certain reference basis for the acquisition,but also need to further improve in the future use.
