| Objective:Using SPSS20.0 and SPSS Modeler18.0 software to conduct data mining on the literature of acupuncture theraphy of hemiplegia last 20 years,explore the rule of selecting acupoints during stroke soft paralysis stage and spastic stage,and guide clinical practice.Methods: Search the literature on acupuncture treatment of limb dysfunction caused by stroke published in the WANFANG MED ONLINE,CNKI,and VIP database from 1998 to 2020,collect the literature based on the inclusion and exclusion criteria,then create database of acupuncture prescription for soft paralysis stage and spastic stage.SPSS Modeler 18.0 and SPSS 20.0 software are used to analyze the data of the two databases in order to find out the rule of selecting points by staged acupuncture.Results: The top five commonly used acupoints during soft paralysis stage are Zusanli(52),Hegu(48),Quchi(43),Neiguan(36),and Jianyu(35).The top three meridians are LI 169(21.13%),ST125(15.63%),and GB114(14.25%).The top three specific acupoints are Wushu acupoint 283(32.60%),crossing acupoint 187(21.54%),and lower He-sea acupoint 113(13.02%).Acupoints are mostly distributed in the limbs.Quchi→Hegu,Hegu→Zusanli,Jianyu→Quchi three groups of points are ranked in the top three in terms of rule support.Chize → Neiguan has the highest confidence.There are four categories of high-frequency acupoints of soft paralysis stage.The first category of acupoints is Xingnaokaiqiao acupoints,the second category of acupoints is belongs to the ST,the third category of acpoints is mostly belongs to the TE and GB,and the fourth category of acupoints is mostly belongs to the LI.The top five commonly used acupoints during spastic stage are Waiguan(40),Jianyu(38),Hegu(35),Yanglingquan(31),Sanyinjiao(30).The top three meridians are LI158(18.02%),TE110(12.54%),and GB 102(11.63%).The top three specific acupoints are Wushu acupoint 236(27.47%),crossing acupoint 209(24.33%),and Yuan-Source acupoint 112(13.04%).Acupoints are mostly distributed in the limbs.The three groups of Yinlingquan→Sanyinjiao,Yinlingquan、Hegu→Sanyinjiao,Hegu、Waiguan→Jianyu are ranked top three in terms of rule support.Yinlingquan→Sanyinjiao,Yinlingquan 、 Hegu→Sanyinjiao,Xuehai 、Yinlingquan→Sanyinjiao,Quchi 、 Hegu 、 Waiguan→Jianyu,have the highest confidence.The high-frequency acupoints of the spastic stage are divided into four categories.The first category of acupoints is mostly belongs to the SP,the second category of acupoints is mostly belongs to the LI,the third category of acupoints is belongs to the BL and SI,and the fourth category of acupoints is mostly belongs to the GB and TE.Conclusion: The acupoint selection in the period of soft paralysis should be based on Xingnaokaiqiao,dredging channels and collaterals,and focus on the use of Xingnaokaiqiao acupuncture points,guided by increasing the muscle tension of the limbs.Choosing points along the meridian,choosing proximal points as well as remote points are the main point selection methods during soft paralysis stage.Combining the same meridian,meridian of the same name,and upper and lower acupoints are the methods of acupoint matching during soft paralysis stage.In the period of spastic stage,the selection of acupoints is based on invigorating qi and nourishing blood,clearing heat and relieving spasms,and relaxing muscles and collaterals.To prevent aggravation of spasticity,and guided by preventing the cramps from getting worse and promoting separation movement.Choosing points along the meridian,choosing proximal points as well as remote points are the main point selection methods during spastic stage.Combining the same meridian,meridian of the same name,and upper and lower points are the methods of acupoint matching during spastic stage.Acupoint selection by stages is more in line with the general rules of motor recovery in hemiplegic limbs after stroke.Research on the rule of selecting acupoints for acupuncture theraphy based on DM technology is worthy of implement. |
