Research objectiveBy observing the clinical effectiveness and safety of the row acupuncture therapy of acupoints of head combined with cervical jiaji in the treatment of mild to moderate vascular dementia after stroke,to deeply discuss the mechanism of action of the therapy,and provide a scientific and reliable theory support and clinical evidence for further promotion and application.Research methodThe cases in this subject were all from the outpatients of the Acupuncture Special Needs Department of the First Teaching Hospital of Tianjin University of TCM.According to the diagnostic,inclusion and exclusion criteria of vascular dementia,finally screened out 60 patients as research objects,and randomly divided into treatment group and control group by random number table method,with 30 cases in each group.All they were given basic treatment:All they received routine symptomatic treatment during the recovery period of stroke,for example,drugs for lowering blood pressure,regulating blood lipids and blood sugar can be taken as prescribed by doctor,to eliminate various interference factors affecting the subject research.Acupoint selection in the treatment group: the Governor Vessel between Shenting and Fengfu,and the 1.5-inch bladder meridian on both sides:the three-line head acupoints were taken at intervals of 1.5 inches to take acupuncture points,and cervical jiaji acupoints(bilateral).Acupoint selection in the control group:Refer to acupoints for treating dementia in the textbook “Acupuncture and Moxibustion”in the“Eleventh Five-Year Plan”of the National Higher Education of Chinese Medicine:Yintang,Shenting,Baihui,Sishencong,Fengchi(bilateral),Zusanli(bilateral),Xuanzhong(bilateral),Taixi(bilateral).Course of treatment:All patients were treated once a day,5 times a week,and 10 times as a course of treatment,a total of 4 courses of treatment.Immediately after enrollment and at the end of the 4th course of treatment,the two groups of patients were evaluated on the Activity of Daily Living scale(ADL),the Mini-Mental State Examination scale(MMSE),the revised Hasegawa Dementia Scale(HDS-R)points and the total efficacy of dementia,to compare the changes in the cognitive function and self-care ability of patients before and after treatment.In the end,used SPSS26.0 statistical software to perform statistical analysis on the obtained data and drew the research conclusion.Research results1.Between the two groups of patients,the baseline data such as gender,age,course of disease,stroke type,education level,and dementia degree were not statistically significant(P>0.05),and were comparable.2.Evaluation of the Mini-Mental State Examination scale(MMSE):Comparing the MMSE scores of two groups before and after treatment,the scores of the two groups before treatment were not statistically significant(P>0.05)and were comparable;the average score of the two groups after treatment was higher than before,and the treatment group was more higher,the scores difference were statistically significant(P<0.05);In both the two groups,the scores before and after treatment were significantly different(P<0.01).3.Evaluation of the Activity of Daily Living scale(ADL):Comparing the ADL scores of two groups before and after treatment,the scores of the two groups before treatment were not statistically significant(P>0.05)and were comparable;the average score of the two groups after treatment was lower than before,and the treatment group was more lower,the scores difference were statistically significant(P<0.05);In both the two groups,the scores before and after treatment were significantly different(P <0.01).4.Evaluation of the revised Hasegawa Dementia Scale(HDS-R):Comparing the HDS-R scores of two groups before and after treatment,the scores of the two groups before treatment were not statistically significant(P>0.05)and were comparable;the average score of the two groups after treatment was higher than before,and the treatment group was more higher,the scores difference were statistically significant(P<0.05);In both the two groups,the scores before and after treatment were significantly different(P <0.01).5.Evaluation of clinical efficacy:Among 30 patients in the treatment group,12 cases were markedly effective,15 cases were effective,and 3 cases were ineffective.The total effective rate of the treatment group was 90%;Among 30 patients in the control group,5 cases were markedly effective,15 cases were effective and 10 cases were ineffective.The total effective rate of the control group was 66.67%.According to statistical analysis,the difference in clinical efficacy between the two groups was statistically significant(P<0.05).6.Safety evaluation: During the study,there were no cases of shedding in the two groups,and there were no unfavorable conditions such as fainting and stuck needles.Research conclusion1.This study shows that: the row acupuncture therapy of acupoints of head combined with cervical jiaji and conventional acupuncture therapy are effective in treating mild to moderate vascular dementia after stroke.The cognitive function,intelligence,and self-care ability of daily life of the two groups of patients have been improved.2.In improving the cognitive function,intelligence and self-care ability of patients,the clinical efficacy of row acupuncture therapy of acupoints of head combined with cervical jiaji is better than conventional acupuncture therapy.3.The row acupuncture therapy of acupoints of head combined with cervical jiaji has a significant effect on mild to moderate vascular dementia,can effectively control the further development of the disease,has no toxic side effects,with high safety.It is suitable for clinical promotion and application.
