| Objective:To evaluate and compare the clinical efficacy of two methods of acupuncture to prevent postoperative urinary retention of mixed hemorrhoids,and to explore the safety of their clinical application,in order to provide a simple,safe and effective way to prevent postoperative urinary retention of mixed hemorrhoids.Methods:In this study,120 patients who were hospitalized in the anorectal department of First Teaching Hospital of Tianjin University of Traditional Chinese Medicine from June 2020 to January 2021 were selected.A total of 120 patients who met the inclusion and exclusion criteria were randomly divided into control group,experimental group A and experimental group B,with 40 patients in each group.The three groups were all given conventional treatment.On the basis of this,acupuncture treatment was given to experimental group A and experimental group B at the second hour after surgery,respectively.The combinations of acupoints selected in the two groups were different.Respectively to observe and record of three groups of patients with gender,age,intravenous fluid infusion on the day of operation(intraoperative and postoperative and total),occurrence of postoperative urinaryretention,urethral catheterization,the first urination after surgery(time,urine volume,urinating condition,the lower abdomen symptoms,micturition scoring),micturition scoring at the 6th hour after surgery,micturition scoring at the 24 th hour after surgery,micturition scoring at the 48 th hour after surgery,postoperative pain of anus,postoperative anxiety status,adverse reaction and safety of acupuncture.Results:1 The three groups of patients were compared in terms of gender,age,and amount of intravenous fluid rehydration on the day of operation(intraoperative,postoperative and total volume).The results showed that the differences were not statistically significant(P >0.05).Patients in the three groups were comparable.2 Three groups of patients were compared in the occurrence of postoperative urinary retention,urethral catheterization,the first urination after surgery(urine volume,urinating condition,micturition scoring),micturition scoring at the 6th hour after surgery,postoperative pain of anus,postoperative anxiety status.The differences were statistically significant(P <0.05).Further pairwise comparisons were made between the groups,and the results showed that between control group and experimental group A or B the difference was statistically significant(P < 0.05).There was no statistically significant difference between experimental group A or experimental B(P > 0.05).3 Three groups of patients were compared in the first urination after surgery(time,the lower abdomen symptoms),micturition scoring at the 24 th hour after surgery,micturition scoring at the 48 th hour after surgery.None of the differences were statistically significant(P > 0.05).4 In this study,none of the patients receiving acupuncture treatment had adverse reactions to acupuncture.Conclusion:1 Acupuncture treatment can effectively prevent the occurrence of urinary retention after mixed hemorrhoids surgery,reduce the incidence of urinary retention and catheterization rate,improve the first postoperative urination to a certain extent,but also reduce the postoperative pain of anus,relieve the anxiety of patients after surgery.2 Whether acupuncture at lower extremities acupoints should be combined with acupuncture at abdominal acupoints to prevent postoperative urinary retention of mixed hemorrhoids has no significant influence on the clinical effect.Therefore,clinical acupoints can be appropriately simplified and only lower extremities acupoints shouid be selected.3 The expeeriment proved that acupuncture therapy is effective and safe to prevent postoperetive urinary retention of mixed hemorrhoids.This technique easy to operate and low-cost.It has clinical application value and is sitable for popularization and use. |
