| Objectives:The purpose of this study was to observe the effect of Warming Yang Huatan Tongluo decoction combined with Tiaoshen Yizhi acupuncture on cognitive function of patients with post-stroke cognitive impairment,and to explore the application of Wen Yang method in post-stroke cognitive impairment,in order to provide more clinical plan and clinical evidence for cognitive impairment after stroke.Methods:This study adopted randomized,controlled trial methods,according to the included and exclusion criteria,selecting patients from June 6,2019-December 2020,who receive treatment in the second affiliated hospital of Tianjin University of Traditional Chinese Medicine encephalopathy inpatient and outpatient.A total of 60 cases were included,by random number table method were randomly divided into treatment group 30 cases and control group 30 cases,the treatment group and control group can be used to treat diseases based drugs,banning the use of drugs to improve intellectual.The control group was treated with donepezil Hydrochloride Tablets combined with Regulating Spirit and Benefiting Intelligence Acupuncture,while the treatment group was treated with Wenyang Huatan Touluo Decoction combined with Regulating Spirit and Benefiting Intelligence Acupuncture for 12 weeks.The simple intelligence scale(MMSE)and Montreal scale(MOCA)were used to evaluate cognitive function,and the ability of daily living(Barthel Index)was used to evaluate activities of daily living.The scores of the treatment group and the control group before and after treatment were compared to evaluate the efficacy of the two groups.The efficacy of TCM syndromes was evaluated according to the score of vascular dementia syndrome differentiation scale(SDSVD).Results:1.During the study,3 patients failed to complete the treatment due to various reasons,and they were cases of shedding.Therefore,57 patients were finally observed,including29 in the control group and 28 in the treatment group.2.There were no significant differences in age,gender,education level,MMSE scale and MOCA scale scores between the two groups before treatment,which were comparable.3.MMSE results: MMSE scores were significantly increased before and after treatment.MMSE scores were compared before and after treatment,among which the scores of total score,attention,calculation ability and language ability were significantly improved,the scores of recall ability were significantly improved,and there were no significant differences in the scores of orientation and memory(instant).4.MOCA results: The comparison of MOCA scores in the control group before and after treatment showed that the scores of total score,visual space and executive ability,naming,attention and delayed recall were significantly increased,the abstract score was significantly increased,and the scores of language ability and orientation were not significantly different.In the treatment group,the scores of total score,visual space and executive ability,attention and delayed recall were significantly increased,the naming score was significantly increased,and the scores of language ability,abstraction and orientation were not significantly different.MoCA scores were compared between the groups before and after treatment,in which the total score and attention score were significantly increased,the recall score was significantly increased,and the other scores were not significantly different.5.ADL results: There was no significant difference in Barthel index scores between the two groups before treatment The differences in Barthel index scores before and after treatment in the two groups were statistically significant.There was no significant difference in Barthel index scores between the two groups after treatment.6.After the end of the study,the TCM syndrome efficacy index of the treatment group was significantly better than that of the control group.Conclusion:1.Donepezil Hydrochloride Tablets combined with Regulating Spirit and Benefiting Intelligence Acupuncture therapy and Wenyang Huatan Touluo Decoction combined with Regulating Spirit and Benefiting Intelligence Acupuncture therapy can improve the cognitive ability of patients with cognitive impairment after stroke.2.In the treatment of cognitive impairment after stroke,Wenyang Huatan Touluo Decoction combined with Regulating Spirit and Benefiting Intelligence Acupuncture is generally superior to the western medicine Donepezil Hydrochloride Tablets combined with Regulating Spirit and Benefiting Intelligence Acupuncture in terms of improving cognitive function,especially in terms of improving computational power and memory of patients.3.Both treatments can improve the ability of daily living of patients,but there is no significant difference.4.The treatment group was better than the control group in improving the overall symptoms of TCM phlegm and stasis And the improvement of phlegm and turbidity obstruction syndrome was more obvious. |
