Objective To compare the efficacy and toxicity of the modified dose-dense chemotherapy with standard 3-weekly regimen as neoadjuvant chemotherapy in advanced epithelia ovarian,fallopian tube and primary peritoneal patients.Methods A retrospective cohort study involving 133 patients diagnosed with stage III/IV epithelial ovarian,fallopian tube or primary peritoneal cancer who received neoadjuvant chemotherapy followed by interval debulking surgery between January 2017 to December 2019 was conducted.patients were divided into modified 1,8 regimen group and standard regimen group according to the administration protocol they received.Patients’ demographic characteristics,chemotherapy delivery,adverse events,surgical outcome and progression-free survival were compared between groups.Results A total of 45 patients in the modified 1,8 regimen group and88 patients in the standard regimen group were eligible for analysis.Among chemotherapy-related adverse events,grade 3/4neutropenia/leukopenia was significantly higher in the 1,8 regimen group than in the standard regimen group(46.7% vs 11.4%;p < 0.05).Although the rate of optimal cytoreduction was similar between groups(82.2% vs78.4%;p = 0.605),we did observe a significant difference in progression-free survival for the modified dose-dense regimen as compared to standard regimen(19.0 months vs 13.5 months;p < 0.05).Multivariate analysis for PFS demonstrated that modified 1,8 regimen represented an independent prognostic factor(p = 0.043;HR = 0.628,95% CI0.401-0.985).Conclusions The modified 1,8 regimen might be a reasonable alternative option of neoadjuvant chemotherapy for advanced epithelia ovarian,fallopian tube and primary peritoneal cancer.
