Clinical Study Of Modified Wendan Decoction In The Treatment Of CKD3-4 Stage With Metabolic Acidosis

Objective:To observe the clinical efficacy of Modified Wendan decoction in the treatment of 3-4 stage of chronic kidney disease with metabolic acidosis(spleen and kidney qi deficiency and damp-heat syndrome).Methods: Observed subjects in this study were all from the outpatients and wards of the Department of Nephrology,the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine.A total of 80 patients who met the inclusion criteria were selected.They were randomly divided into treatment group(40 cases)and control group(40 cases).The treatment group was treated with Modified Wendan decoction on the basic treatment,and the control group was treated with Shenshuaining tablet on the basic treatment.The treatment period was 4 weeks.The improvement of TCM syndrome and the changes of serum creatinine,serum urea nitrogen,glomerular filtration rate,carbon dioxide binding capacity and serum ions were observed and compared between the two groups before and after treatment.Results:1.The total effective rate of the treatment group in this observation was85.00%,and the total effective rate of the control group was 76.32%,and the treatment group was superior to the control group(P< 0.05).2.After treatment,the total scores of TCM syndromes and the scores of various TCM syndromes in the two groups were lower than those before treatment.the treatment group was superior to the control group in improving symptoms such as fatigue,weakness of waist and knees,anorexia,nausea and vomiting,dry mouth and bitter mouth,sticky mouth and false stool(P< 0.05).There was no significant difference in the improvement of limb tiredness and epigastric distension between the treatment group and the control group(P>0.05).3.The serum Serum carbon dioxide binding of the two groups after treatment was higher than that before treatment,and the treatment group was superior to the control group(P< 0.05).4.The serum creatinine,serum urea nitrogen and glomerular filtration rate of the two groups were improved after treatment,and the treatment group was better than the control group(P< 0.05).Conclusion:1.Modified Wendan decoction can significantly improve the clinical symptoms of chronic kidney disease stage 3-4 with metabolic acidosis(spleen and kidney qi deficiency and damp-heat syndrome),such as fatigue,sore waist and knees,lack of food,nausea and vomiting,dry mouth and bitter mouth.2.Modified Wendan decoction have a certain advantage in correcting chronic kidney disease stage 3-4 with metabolic acidosis(spleen and kidney qi deficiency and damp-heat syndrome),raising carbon dioxide binding.3.Modified Wendan decoction can reduce the level of serum creatinine and serum urea nitrogen in patients with chronic kidney disease in stage 3-4with metabolic acidosis(spleen and kidney qi deficiency and damp-heat syndrome),and improve glomerular filtration rate.
