Objective:To study the influence of insulin resistance and body weight on the health related quality of life questionnaire(Chi-PCOSQ)score of polycystic ovary syndrome,and to explore the difference of quality of life of PCOS patients with different IR status,so as to provide the basis for personalized treatment and psychological intervention of PCOS patients.To observe the effect of auricular acupoint seed pressing treatment on Chi-PCOSQ score of overweight/obese PCOS patients with IR,and compare the effects of different courses of treatment on the quality of life of PCOS patients,so as to provide clinical evidence for auricular seed pressing to improve the quality of life of PCOS patients,and to provide scientific basis for auricular seed pressing as a non-drug therapy of traditional Chinese medicine for the treatment of PCOS.Methods:Part I: Cases were collected from PCOS cases who filled in the integrated electronic system of clinical research information in the scientific research outpatient department of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from January 2018 to December2019.The general clinical characteristics(BMI,weight,height,age)of the patients were collected by retrospective analysis.Biochemical indicators(fasting insulin,fasting blood glucose,HOMA-IR);Part 2: This study was approved by the Ethics Committee of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine.The patients were from PCOS patients recruited in the department of gynaecology of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from January 2018 to October 2020,and they were randomly divided into treatment group(treatment method was ear acupoint seed compression combined with lifestyle improvement,n=35)and the control group(the intervention method was lifestyle improvement,n=35),the intervention time was 6 months,the general data of patients,before treatment,3 months after treatment and the end of treatment were collected.SPSS 21.0 statistical software was used for statistical analysis of the collected data.Results:1.A total of 249 PCOS patients were collected in the first part of the study.The total score of Chi-PCOSQ was 21.47 ± 3.86,and the order of scores of various dimensions of Chi-PCOSQ was: infertility score <weight dimension score <body hair dimension score <menstrual disorders dimension score<emotional dimension score.A total of 249 PCOS patients were collected in the first part of the study.The total score of score<emotional dimension score.2.Influence of insulin resistance on Chi-PCOSQ in patients with PCOSIn the non-IR group(n=66)and IR group(n=183),the body hair dimension score,body weight dimension score and total score of Chi-PCOSQ of PCOS patients without insulin resistance were significantly higher than those of PCOS patients with insulin resistance(P<0.05),and there was no statistical significance in scores of emotional dimension,infertility dimension and menstrual disorder between the two groups(P>0.05).3.Influence of BMI on Chi-PCOSQ in PCOS patientsIn the low BMI group(n=94 cases)and the high BMI group(n=155cases),the scores of emotional dimension,body hair dimension,weight dimension,infertility dimension and total score of Chi-PCOSQ in the low BMI group were significantly higher than those in the high BMI group(P<0.05).There was no statistically significant difference in the score of menstrual disorders between the two groups(P>0.05).4.Effect of auricular acupoint seed pressing therapy on Chi-PCOSQ in overweight/obese patients with PCOS complicated with IRAfter 3 months of treatment,scores of emotional dimension and weight dimension in auricular acupoint seed pressing group were significantly higher than those in control group,and the differences were statistically significant(P<0.05);After 6 months of treatment,the scores of emotional dimension and weight dimension in auricular acupoint seed pressing group also significantly increased the scores of menstrual disorder dimension(P<0.05)).After 6 months of ear acupoint seed pressing treatment,the scores of emotional dimension,body hair dimension,weight dimension,infertility dimension and menstrual disorder dimension in the treatment group were higher than before.Conclusion:1.IR and BMI have influences on the scores of various dimensions of Chi-PCOSQ in PCOS patients.2.The body hair,body weight and total score of Chi-PCOSQ in PCOS patients without IR were significantly higher than those in PCOS patients with IR.3.The scores of emotional dimension,body hair dimension,weight dimension,infertility dimension and total score of Chi-PCOSQ in the low BMI group were significantly higher than those in the high BMI group.4.In clinical treatment,individualized treatment plans should be formulated for PCOS patients according to the levels of IR and BMI of PCOS patients.5.Ear acupoint seed pressing can effectively improve the score of all dimensions of Chi-PCOSQ of PCOS patients,and effectively improve their quality of life.6.The 6-month treatment course of ear acupoint seed pressing was better than the 3-month treatment course,indicating that the length of treatment course of ear acupoint seed pressing had a significant impact on the quality of life of PCOS patients.
