| Objective:To investigate the safety and effectiveness of intraoperative application of lobaplatin in the prevention of postoperative tumor recurrence of gastric cancer.Methods:A randomized controlled study design was used to study 80 inpatients who were diagnosed with advanced gastric cancer in the abdominal surgery department of the Affiliated Cancer Hospital of Nanchang University from June 2018 to June 2019.The control group(40 cases)received laparoscopic D2 radical gastrectomy,and the experimental group(40 cases)received laparoscopic D2 radical gastrectomy,during which 100 mg loplatin was diluted in 250 ml warm normal saline and the abdominal drainage tube was clamped for 6h.During the operation,abdominal flushing fluid was taken from both groups for the detection of cancerous cells.Fast-fasting blood samples were collected before surgery and on the 1st,3rd and 5th day after surgery to detect liver and kidney function and blood routine tests.Postoperatively,the patients’ vital signs,gastrointestinal ventilation time,toxic reactions and surgical complications were observed.Tumor markers CEA,CA125,CA199,CA724 and tumor recurrence were followed up for 1 and 2 years.Results:Both groups of patients successfully completed D2 radical gastrectomy of gastric cancer,and the postoperative complication rates in the experimental group and the control group were 17.5% and 15%,respectively,with no statistically significant complications including pulmonary infection,abdominal infection,intestinal obstruction and incision infection.There was no statistical significance in the indexes of liver and kidney function,gastrointestinal ventilation time,hospitalization time and intraoperative cancerous cells of the experimental group and the control group on the1 st,3rd and 5th day after surgery(P > 0.05).The leukopenia and thrombocytopenia in the control group were statistically significant(P < 0.05).Postoperative tumor markers CA125,CA199 and CA724 showed no statistical significance,while CEA results of tumor markers 1 and 2 years after surgery showed statistical significance(P< 0.05).In the experimental group,the recurrence rate was 5% one year after operation.The recurrence rate was 10% in the control group and 22.5% in the experimental group.In the control group,the recurrence rate was 37.5% 2 years after operation.The recurrence rates of patients in 2 groups at 1 and 2 years after surgery were statistically significant(P < 0.05).Conclusion:Lobaplatin intraperitoneal lavage chemotherapy is safe and effective in the prevention of postoperative recurrence of gastric cancer. |
