Chronic actinic dermatitis(CAD)is a group of UV-related spectral diseases characterized by chronic photosensitivity in middle-aged and elderly men.The clinical manifestations were recurrent chronic dermatitis mainly in the exposed site.Its pathogenesis is not fully understood at present.This study aims to observe the minimum erythema dose(MED)in patients with CAD,detect the levels of IL-6 and IL-8 in peripheral blood of CAD patients and healthy people,and explore the correlation between IL-6 and the incidence of CAD.To observe the clinical efficacy of hydrochloroquine(HCQ)combined with tripterygium glycosides(TWG)in the treatment of CAD,and to explore the possible mechanism of action.A total of 56 CAD patients in the dermatology outpatient department and wards of the First Affiliated Hospital of Hebei North University were selected from January 2019 to February 2021,and 58 healthy subjects who underwent physical examination at the hospital of the physical examination center were selected as the normal control group during the same period.The minimum amount of erythema MED was determined in all subjects.The levels of IL-6 and IL-8 in the peripheral blood of CAD patients and healthy subjects were detected respectively.The patients with CAD were randomly divided into three groups: group A with hydroxychloroquine,group B with Tripterygium glycosides,and group C with combined treatment.Levetirizine hydrochloride tablets were taken orally,5mg each time,once a night.Nide cream,twice a day;Vitamin E cream,twice a day.Group A was given 100 mg hydroxychloroquine twice A day,and group B was given Tripterygium wilfordii polyglycoside tablets20 mg every time,2 times A day.After 4 weeks of oral treatment,the levels of IL-6 and IL-8 in CAD patients were measured again.The therapeutic effect of the three groups,the levels of IL-6 and IL-8 in normal control group and CAD patients before and after treatment were compared.Before and after treatment,CAD patients were assessed according to the Eczema Area and Severity Index(EASI)scoring method,and the Total Integral Descent Index(TIDI)was calculated after treatment.SPSS 22.0 statistical software was used to input and analyze the data.The results showed that :(1)both UVA-MED and UVB-MED in CAD patients were lower than those in normal control group,and the difference was statistically significant(P<0.01).There was no significant difference between type Ⅲ skin UVA-MED and type Ⅳ skin UVA-MED in CAD group(P > 0.05).The UVA-MED of type Ⅲ skin was lower than that of type Ⅳ skin in normal control group(P< 0.05).The UVB-MED of typeⅢ skin was lower than that of type Ⅳ skin in CAD patients and normal control group(P< 0.05).(2)The Eczema Area and Severity Index(EASI)were significantly decreased in all CAD groups after treatment(P < 0.01).The curative effect of the group C was higher than that of the group B and A(P < 0.05),and there was no difference in the effective rate between group A and B(P >0.05).After 8 weeks of treatment,the recurrence rate of group C was lower than that of group B and A(P < 0.05),and there was no difference in the recurrence rate between group A and B(P >0.05).(3)The levels of the IL-6 and IL-8 in the CAD patients were significantly higher than those in the normal control group(P< 0.01);And after the treatment,the levels of the IL-6 and IL-8 in the three groups were significantly lower than before treatment(P< 0.01).These results indicate that :(1)The minimum erythema volume(MED)of CAD patients is lower than that of normal control group,and the incidence of CAD is related to UVA and UVB in ultraviolet rays.(2)The levels of IL-6 and IL-8 in CAD patients were higher than those in the normal control group,suggesting a certain relationship between the incidence of CAD patients and the increase of IL-6 and IL-8.(3)Hydroxychloroquine,Tripterygium wilfordii polyglycosides and their combination therapy were all effective in CAD patients,and the combination therapy had better effect.(4)The therapeutic effect of hydroxychloroquine,tripterygium glycosides and their combination on CAD patients may be related to the regulation of the secretion of IL-6 and IL-8.
