with diffuse toxic goiter with hyperactivity of liver fire who were admitted to the Department of Endocrinology of Qinhuangdao Hospital of Traditional Chinese Medicine from June 2019 to July 2020 meeting the inclusion criteria were selected and divided into control group and treatment group according to random number table method.Control group: Methimazole was given orally,once a day,5-30mg/day according to the severity of the disease,30 days as a course of treatment,and the treatment lasted for 6 courses.Treatment group: on the basis of the control group,the Xiaoyaozhuanalgesic ointment was applied locally to the thyroid area from 9 o ’clock every night to 7 o ’clock the next morning,once a night,for 25 days,with a rest of 5 days,30 days as a course of treatment,for 6 courses of treatment.Basic vital signs,blood routine,liver and kidney function,electrocardiogram,thyroid volume,degree of goitre,thyroid function,thyroid stimulating hormone receptor antibody(TRAb),TCM syndrome quantification score,TCM single symptom score,etc.,before and after treatment in 2 groups were recorded,and statistical analysis was performed using SPSS 26.0 software.Results(1)There was a statistically significant difference in thyroid volume between the control group and the treatment group before and after treatment(P<0.01),and a statistically significant difference between the two groups after treatment(P<0.01).(2)There was statistically significant difference in the degree of goiter between the control group and the treatment group before and after treatment(P<0.01).After treatment,there was statistically significant difference between the two groups(P<0.01).(3)There were statistically significant differences in triiodothyrogenine(FT3),thyroxine(FT4)and thyrotropin(TSH)between the two groups before and after treatment(P<0.01).There was no statistical difference between the two groups after treatment(P<00.05).(4)There was a significant difference in TRAb between the two groups before and after treatment(P<0.01).There was no statistical difference between the two groups after treatment(P<00.05).(5)There was a statistical difference in TCM syndrome scores in the control group before and after treatment(P<0.05),and there was a statistically significant difference in TCM syndrome scores in the treatment group(P<0.01).After treatment,there was a significant difference between the two groups(P<0.01).(6)There were significant differences in TCM symptom scores between the two groups before and after treatment(P<0.05).After treatment,there were statistical differences in anterior neck gall swelling,neck distention and discomfort,irritability,dry mouth and bitter mouth,red eyes and tongue pulse between the two groups(P<0.05),while there were no statistical differences in weight loss and palpitation and restlessness between the two groups(P>0.05).(7)Efficacy evaluation of the disease: in the treatment group of 33 cases,3 cases were cured clinically,11 cases were significantly effective,15 cases were effective,and 4 cases were ineffective,with an effective rate of87.90%.In the control group of 33 cases,0 cases were cured,8 cases showed obvious effect,12 cases were effective,13 cases were ineffective,and the effective rate was 60.60%.There was statistical difference between the two groups after treatment(P<0.05).(8)Efficacy evaluation of TCM syndrome integral: in the treatment group of 33 cases,3 cases were cured clinically,8 cases were significantly effective,19 cases were effective,and 3 cases were ineffective,with an effective rate of 90.90%.In the control group(33 cases),0 cases were cured,6 cases showed obvious effect,17 cases were effective,and 10 cases were ineffective.The effective rate was 69.70%.After treatment,there was a significant difference between the two groups(P<0.01).Conclusion(1)The combination of Xiaoyongzhonganalgesic ointment and methimazole group was superior to the single methimazole group in reducing thyroid volume and improving goiter.(2)Compared with the methimazole alone group,the combination of methimazole ointment and methimazole ointment had the same effect in improving thyroid hormone levels and TRAb.(3)Xiaoyingzhongzhitong ointment combined with methimazole group was better than methimazole group in improving TCM syndrome quantification score and syndrome efficacy.(4)The combination of methimazole and xiaoyingzitong ointment was better than methimazole alone in improving the symptoms of neck front gall,neck distention and discomfort,irritability,dry mouth,bitter mouth,red face,red eyes and tongue pulse,and the two groups had the same effect in improving weight loss and palpitation and restlessness. |
