Effect Of Kuijieshuang On Levels Of Inflammatory Factors In Patients With Cold-heat Mixed Ulcerative Colitis

Objective: To observe the clinical efficacy of Kuijieshuang in the treatment of cold-heat mixed mild to moderate ulcerative colitis,and to explore its therapeutic mechanism through the changes of serum inflammatory cytokines IL-6 and IL-8 levels before and after treatment.Methods: A total of 70 patients with mild to moderate ulcerative colitis with mixed cold and heat who were admitted to the Spleen and Gastric Department of Shijiazhuang City Hospital of Traditional Chinese Medicine from October 2019 to October 2020 and met the inclusion criteria were selected.The patients were randomly divided into treatment group and control group,35 patients in each group were treated with Kuijieshuang and Mesalazine,respectively.For both groups,4 weeks was a course of treatment,and 3 courses of treatment were observed.Recording the clinical curative effect of two groups before and after treatment,TCM syndrome integral change(including diarrhea purulent blood,stool mass thin hot,abdominal pain,diarrhea,anus,chills joy or afraid of the cold,thirsty to drink hot drinks,abdominal signs),TCM syndrome curative effect,colonoscopy under effect,expression of inflammatory cytokines in serum(IL-6,IL-8),laboratory examination indexes and the information such as the safety index monitoring during treatment,and record the drug allergy happened and side effects.Finally,statistical software SPSS26.0was used for statistical analysis of all the observed data.The differences of the results between the two groups were compared.Results: 1 Comparison of clinical efficacy: after treatment,the effective rates of the treatment group and the control group were 93.75%and 87.50%,respectively,with statistical difference between groups(P<0.05).2 Comparison of total scores of TCM syndromes: there were significant differences within groups(P<0.01);There was statistical difference between groups(P<0.05).3 Comparison of scores of TCM syndromes: there were statistical differences within the groups(P<0.05);There were statistically significant differences in the frequency of diarrhea,anal burning,continuous abdominal pain,thirst not to drink or like hot drinks,and abdominal burning feeling between groups(P<0.05).There was no significant difference in dysentery,pus and blood,fecal thinness and fear of cold(P>0.05).4 Comparison of the efficacy of TCM syndromes:after treatment,the effective rates of the treatment group and the control group were 90.63% and 87.50%,respectively,with statistical difference between groups(P<0.05).5 Comparison of efficacy under colonoscopy:after treatment,the effective rates of the treatment group and the control group were 84.38% and 81.25%,respectively,and there was no statistical difference between the two groups(P>0.05).6 Comparison of the expression of inflammatory cytokines in serum: serum IL-6 levels were significantly different among all groups(P<0.01);There was statistical difference between groups(P<0.05).The serum IL-8 level was significantly different among all groups(P<0.01).There was statistical difference between groups(P<0.05).7 Comparison of laboratory indicators:there were significant differences in WBC count,neutrophil absolute value,erythrocyte sedimentation rate,WBC count and RBC count in stool routine among groups(P<0.01).There was no significant difference in hemoglobin among groups(P>0.05).There were no significant differences in WBC count,absolute value of neutrophils,hemoglobin,erythrocyte sedimentation rate,WBC count and RBC count in stool routine among groups(P>0.05).8 Monitoring of safety indicators: No abnormalities were found in the results of safety indicators before and after treatment.Conclusion: 1 Kuijie Shuang treats mild to moderate ulcerative colitis with mixed cold and heat,which can relieve patients diarrhea,mucous pus,blood stool,abdominal pain and other clinical symptoms,and improve patients TCM syndromes;2 Kuijie Shuang can promote the improvement of intestinal mucosal lesions of mild and moderate ulcerative colitis with mixed cold and heat.3 One of its mechanisms may be through reducing the levels of inflammatory cytokines IL-6 and IL-8 in serum.4 It is safe and reliable,worthy of further study and clinical application.
