Objective:To explore the correlation between blood routine indexes and the pathogenesis and efficacy of sudden hearing loss patients with different types,from the count of peripheral blood index,including leukocyte count,neutrophil count,lymphocyte count,platelet count,mean platelet volume and neutrophils-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),and then provide guidance for the clinical diagnosis and treatment and prognosis evaluation of patients with sudden hearing loss.Methods:In this study,430 patients with sudden hearing loss admitted to our hospital from January 2017 to December 2020 were selected as the case group,and 100 patients admitted to the Otolaryngology Department during the same period without a history of middle ear or inner ear diseases were selected as the control group.The diagnosis and treatment of the case group all met the criteria of《Guidelines for Diagnosis and Treatment of Sudden Deafness(2015)》.Peripheral blood was collected from both groups of subjects within 24 hours after admission for routine blood tests.Analysis indicators included: leukocyte count,neutrophil count,lymphocyte count,platelet count,mean platelet volume,neutrophil-to-lymphocyte ratio(NLR)and platelet-to-lymphocyte ratio(PLR).The pure tone threshold was measured before and after treatment in the case group,and the efficacy was recorded.Patients with sudden hearing loss were divided into groups according to different hearing threshold curves and different efficacy,and blood routine indexes were compared.Results:(1)Comparison of blood routine indexes between the case group and the control group,the leukocyte count,neutrophil count,platelet count and neutrophil-to-lymphocyte ratio(NLR)in peripheral blood of the case group were all higher than those of the control group,and the difference was statistically significant(P<0.05),and there were no significant differences in lymphocyte count,platelet volume and platelet-to-lymphocyte ratio(PLR)between the two groups(P>0.05).(2)Comparison among groups of patients with different types of sudden hearing loss showed that the platelet count of patients with high frequency type was higher than that of patients with other types of sudden hearing loss,but the different was statistically significant only with patients with total deafness type(P<0.05),and there was no statistical significance in other indicators among all groups.(3)There was no statistical significance in blood routine indexes among patients with sudden hearing loss with different therapeutic effects(P>0.05).(4)The comparison of blood routine indexes of different curative effects in patients with different types of sudden hearing loss showed that PLT in the total effective group was higher than that in the ineffective group in patients with low frequency sudden hearing loss,and the difference was statistically significant(P<0.05).In patients with high frequency sudden hearing loss,PLR of the total effective group was higher than that of the ineffective group,and the difference was statistically significant(P<0.05).There was no statistical significance between the other blood routine indexes and the curative effect of all types of sudden hearing loss(P>0.05).Conclusion:(1)Leukocyte count,neutrophil count,platelet count and neutrophil/lymphocyte ratio(NLR)in the case group were higher than those in the control group,suggesting that inflammatory reaction plays a role in the pathogenesis of patients with sudden hearing loss,and the disturbance of inner ear microcirculation caused by inflammatory reaction may be one of the pathogenesis of sudden hearing loss.(2)PLT may be correlated with the prognosis of patients with low frequency sudden hearing loss,and PLR may be correlated with the prognosis of patients with high frequency sudden hearing loss.
