| BackgroundAt present,the incidence of sudden deafness is high,and there is no specific treatment drugs and methods for sudden deafness,so it is urgent to seek a new treatment strategy for sudden deafness.Sound therapy for tinnitus is widely carried out around the world and has achieved good results.It is suspected that sound therapy may be effective in the treatment of sudden deafness.ObjectiveTo investigate the clinical effect of noninvasive acoustic stimulation therapy on different types of sudden deafness,we evaluate the therapeutic effect according to the2015 guideline,and discuss the prognostic factors,so as to provide new methods and reference for clinical treatment.MethodsA total of 379 patients with sudden deafness who received inpatient treatment in the Department of Otolaryngology,Head and Neck Surgery of the First Affiliated Hospital of Guangzhou Medical University from January 2017 to October 2020 were selected.Among them,223 patients received sound therapy plus drug therapy,and156 patients received drug therapy.According to the hearing curve of patients at admission in the 2015 guidelines for diagnosis and treatment of sudden deafness,they were divided into:(1)low frequency decline type,(2)high frequency decline type,(3)flat type and(4)total deafness type.We compare the effective rate of two groups of patients in different types of hearing loss.Clinical data of 223 patients receiving sound therapy with drug therapy group were collected.Including gender,age,height,weight,course of the disease,season(spring,summer,autumn,winter),respiratory virus antibody(negative and positive),with types of affected ears(left ear,right ear,both ears),tinnitus cases(no tinnitus,unilateral tinnitus,double tinnitus),degree of hearing loss(mild,moderate,moderately severe,severe and very severe),the types of hearing loss,hematology indexes(PT,FIB,PTR,APTT,TT,CHOL,TG,HDL,LDL,IgG,IgA,IgM,complement C3,complement C4,CH50,microglobulin,ceruloplasin)and other factors that may be related to the efficacy of sudden deafness were analyzed statistically to explore the influence of the above factors on the efficacy of sudden deafness.Results1.Statistics of the effective rate of patients:the total effective rate of the acoustic treatment group was 41.3%,the total effective rate of the drug group was 30.1%,χ~2=4.894,P=0.027,P<0.05,there was a significant statistical difference.1.1 In the low-frequency decreasing type,the cure rate of the acoustic stimulus group was 34.5%,and the total effective rate was 51.7%,while that of the drug group was 53.3%,and the total effective rate was 56.6%.The effective rate between the groups was analyzed by Fisher’s exact test,χ~2=7.747,P=0.052,and P>0.05,showing no statistically significant difference.1.2 In the high-frequency decreasing type,the cure rate of the acoustic stimulus group was 11.8%,and the total effective rate was 32.4%,while that of the drug group was 10%,and the total effective rate was 20%.The effective rate between the groups was analyzed by Fisher’s exact test,χ~2=4.099,P=0.251,and P>0.05,showing no statistically significant difference.1.3 In the flat type,the cure rate of the acoustic stimulus group was 21.7%,the total effective rate was 41.7%,while the cure rate of the drug group was 4.1%,the total effective rate was 24.5%.The effective rate between the groups was determined by Fisher’s exact test,χ~2=28.168,P=0.04,P<0.05,showing a statistically significant difference.1.4 In the total deafness type,the cure rate was 40.5%and the total effective rate was 40.5%in the acoustic stimulus group,while the cure rate was 0%and the total effective rate was 27%in the drug group.The effective rate between the groups was analyzed by Fisher’s exact test,χ~2=35.168,P<0.01,showing a statistically significant difference.2.Analysis of related factors affecting the prognosis of sudden deafness2.1 Age,course of the disease,types of affected ears,degree of hearing loss and FIB are related to the curative effect of sudden deafness.2.2 Gender,BMI,onset season,antibody results of respiratory virus,tinnitus cases,degree of hearing loss,PT,PTR,APTT,TT,CHOL,TG,HDL,LDL,IgG,IgA,IgM,complement C3,complement C4,CH50,microglobulin,ceruloplasin had no correlation with the efficacy of sudden deafness.Conclusion1.The effect of sudden deafness with low frequency decline was the best,followed by flat type and total deafness type,and the worst with high frequency decline type.2.The effective rate of acoustic therapy with adjuvant drugs in the treatment of sudden deafness was better than that of the single drug treatment group,especially in the flat type and total type sudden deafness.3.Age,course of the disease,types of affected ears,degree of hearing loss and FIB are the impression factors affecting the curative effect of sudden deafness.4.Age over 69 years old and the course of disease over 14 days were the main factors affecting the efficacy of sudden deafness. |
