| Objective Based on resting state functional magnetic resonance imaging(f MRI),regional consistency(ReHo)analysis was used to investigate the differences of regional brain function between patients with treatment-resistant depression and healthy controls.Objective to analyze the changes of regional brain functional activity in patients with treatment-resistant depression(TDR)before and after MECT treatment,and take the brain regions with significant difference in ReHo value before and after MECT treatment as the regions of interest.Further,based on the whole brain functional connectivity analysis method,the changes of functional connectivity between brain regions in patients with treatment-resistant depression before and after modified electroconvulsive therapy were studied,and explore the correlation between these functional changes and the improvement of clinical symptoms,so as to explore its significance.Methods A total of 15 patients with treatment-resistant depression and 15 matched healthy controls were included in this study.All patients with treatment-resistant depression completed MECT treatment(treatment times ≤ 12 times).General demographic data were collected to evaluate the Hamilton Depression Scale(HAMD)before and after treatment in the depression group and in the control group at baseline,The reduction rate of depression scale was calculated.The data of functional magnetic resonance imaging(f MRI)before and after MECT treatment in depression group and control group were collected.DPARSF5.1 and spm12 kits were used to compare the differences of regional brain function between depression group and healthy control group and before and after MECT treatment.The seed points were the brain regions with differences before and after MECT treatment,and the whole brain functional connectivity analysis method was further used,Objective to analyze the changes of functional connectivity between brain regions in patients with treatment-resistant depression before and after modified electroconvulsive therapy.Result Compared with the control group,the score of right superior occipital gyrus and right superior occipital gyrus in the treatment group were significantly lower(P < 0.05).In the depression group,ReHo of left middle frontal gyrus after MECT treatment was higher than that before treatment.This area was extracted as the region of interest and used as the seed point to analyze the whole brain functional connectivity.It was found that compared with before MECT treatment,the functional connectivity of left middle frontal gyrus,left temporal lobe,left anterior cingulate gyrus,posterior cingulate gyrus,left frontal lobe,left precuneus and right angular gyrus in TRD patients was enhanced.Conclusion There are regional functional abnormalities in treatment-resistant depression,mainly including the decrease of ReHo value in right posterior central gyrus and the increase of ReHo value in right superior occipital gyrus and left angular gyrus.After MECT treatment,the ReHo value of the left middle frontal gyrus in TRD patients increased,and the functional connectivity between the left middle frontal gyrus(the area with the increased ReHo value)and the left temporal lobe,the left anterior cingulate gyrus,the left posterior cingulate gyrus,the left frontal lobe,the left precuneus lobe and the right angular gyrus increased.These changes may be related to the antidepressant mechanism of MECT. |
