| Objective:Contrast and research the effect of hyperthermia combined with concurrent chemoradiotherapy on serum VEGF expression level and its clinical significance in patients with middle-advanced cervical squamous cell carcinomas,and the occurrence of side effects of radiotherapy and chemotherapy,analyze the effect of hyperthermia on increasing the curative effect of concurrent chemoradiotherapy in the treatment of middle-advanced cervical squamous cell carcinoma and alleviating the side effects of chemoradiotherapy,analysis and discuss the value of serum VEGF expression level in clinical diagnosis and treatment 、 evaluate and predict the curative effect of middle-advanced cervical squamous cell carcinoma.Methods:According to the inclusion criteria of this study,A total of 60 patients(Figo2018 is divided into phase II~IVA)with middle-advanced cervical squamous cell carcinoma diagnosed by case were selected from the radiotherapy department of our hospital from January 2019 to December 2020(The patients ranged in age from 35 to82 years,and all were newly treated patients),They were randomly divided into study group and control group by random number table method,with 30 patients in each group.Before treatment,age,clinical stage,lymph node metastasis,KPS score and other general conditions were compared between the two groups.The differences were not statistically significant(p>0.05).Before treatment,5ml of fasting elbow venous blood was extracted from both groups,serum samples were separated after natural coagulation at room temperature and centrifuge at 2000r/min for 10 min,the supernatant was frozen at-80℃ for testing.The experimental group was given hyperthermia combined with concurrent chemoradiotherapy when the control group was only given concurrent chemoradiotherapy.The radiation treatment is Intensity-modulated radiation therapy combined with brachytherapy and isplatin monotherapy was used in chemotherapy.Hyperthermia: Complete pelvic hyperthermia was performed with Hokai HG-2000 I external high-frequency hyperthermia machine,the diameter of the front plate and rear plate of the radiator are both 20 cm and the temperature is 40~43℃,the heating time is 40 min,2 times per week for 5weeks,heating therapy was initiated 1 week after external irradiation.In both groups,5ml of fasting elbow vein blood was extracted again within one week after the end of treatment,after centrifugation,the supernatant was stored for testing.The serum VEGF expression level was detected by double antibody sandwich enzyme linked immunosorbent assay before and after treatment,respectively.Compare the changes of serum VEGF in 2 groups and the incidence of adverse reactions of radiotherapy and chemotherapy,such as radiation enteritis,radiation cystitis and leukopenia.Results:1.Comparison of serum VEGF expression levels before and after treatment:After treatment,the serum VEGF level of patients with cervical cancer in the experimental group and control group was significantly lower than that before treatment,the difference was statistically significant(p<0.05);2.Comparison of changes in serum VEGF expression level between the two groups after treatment:The level of serum VEGF in the two groups was consistent before treatment,and the difference was not statistically significant(p=0.882>0.05);After treatment,the serum VEGF level in the experimental group was significantly lower than that in the control group,the difference was statistically significant(p<0.05);In other words,the serum VEGF level in the experimental group decreased more significantly than that in the control group after treatment.3.Comparison of the incidence of side effects of radiotherapy and chemotherapy between the two groups after treatment:After treatment,the incidence of radiation enteritis,radiation cystitis and leukopenia in patients with cervical cancer in the experimental group was lower than that in the control group,the difference was statistically significant(p<0.05).Conclusions:1.The expression level of serum VEGF in patients with middle-advanced cervical squamous cell carcinoma was significantly decreased after hyperthermia combined with concurrent radiotherapy or concurrent radiotherapy,It can be used as another index to evaluate the curative effect of cervical cancer.2.Hyperthermia combined with chemoradiotherapy in the treatment of middle-advanced cervical squamous cell carcinoma can reduce the expression level of serum VEGF more than chemoradiotherapy,It is suggested that hyperthermia can increase the curative effect of concurrent chemoradiotherapy in the treatment of middle-advanced cervical squamous cell carcinoma.3.Hyperthermia can reduce the incidence of side effects such as radiation enteritis,radiation cystitis and leukopenia in patients with middle-advanced cervical squamous cell carcinoma. |
