Objective:The study to investigate the Hcy of level in blood and the degree of coronary artery disease in patients with H type hypertension and ACS.Methods:From September 2018 to January 2020,90 patients with H-type hypertension complicated and coronary syndrome admitted to Department of Cardiology,Fourth Affiliated Hospital of Inner Mongolia Medical University were selected as the experimental group, and 90 patients with general hypertension complicated and coronary syndrome admitted selected as the control group,The average age was 67.40 ±30 years old;The number of male patients and female patients in the control group were 55 and 35 respectively, the minimum age was 42 years old,the maximum age was 84 years old,the average age was 67.37 ±13 years old. After approval, all subjects underwent clinical data collection(age, gender, smoking history, cardiac and renal function indexes, etc.)and coronary angiography, and plasma levels of HDL(high density lipoprotein),LDL(low density lipoprotein),TC(cholesterol),TG(triglyceride)and Hcy(homocysteine)were determined,According to the three levels of low, medium and high, the patients were divided into three groups:41 patients in low-risk group(SYNTAX score 0-22),26 patients in medium risk group(SYNTAX score 23-32),23 patients in high-risk group(SYNTAX score ≥ 33),and 90 patients in control group with general high blood pressure and coronary syndrome. All the test data and evaluation data were collected, and the relevant data were measured and analyzed by SPSS17.0 statistical software±Standard deviation(),the comparative analysis between different groups is mainly based on variance F-test,Pearson analysis is used to analyze the correlation between the two variables, and multiple linear regression analysis is used to comprehensively analyze the correlation factors of Hcy level. If the measured score is consistent with P<0.05,it can be judged to be statistically significant.Results:(1)Age,gender and other basic information, systolic blood pressure, smoking history, history of diabetes mellitus, family history of hypertension, history of hyperlipidemia, and types of ACS were compared in each group.(2)There was no significant difference in BMI among the three groups of patients in the experimental group and the control group(P>0.05).The triglyceride and cholesterol values in the high-risk group were significantly higher than those in the low-risk group, the medium risk group and the control group, and the high-density lipoprotein values in the high-risk group were significantly lower than those in the medium risk group, the low-risk group and the control group,The LDL of high risk group was significantly higher than that of medium risk group, low risk group and control group(3)The myocardial function and renal function parameters of different groups were compared and analyzed α-HBD,LDH,AST,CK and CK-MB were significantly higher in moderate risk group, low risk group and control group; The level of troponin in high-risk group was also significantly higher than that in medium risk group,low-risk group and control group; The level of B-type natriuretic peptide(BNP)in high risk group was significantly higher than that in medium risk group, low risk group and control group;Meanwhile, the levels of C-reactive protein(CRP)and procalcitonin(PCT)in high-risk group were significantly higher than those in medium risk group,low-risk group and control group;Compared with renal function parameters, serum creatinine(SCR)and urea nitrogen(BUN)in high-risk group were significantly higher than those in medium risk group,low-risk group and control group.(4)The SYNTAX score in the high-risk group of H type was significantly higher than that in the low-risk group of H type hypertension, and with the increase of Hcy concentration, the SYNTAX score was higher.(5)The independent variable factors of different groups of patients in syntax integral process can affect the dependent variable Hcy homocysteine. In the implementation of multiple linear regression analysis, the integral variables corresponding to the age and coronary artery disease of four groups of patients can have a significant impact on the level of Hcy,which has no significant relationship with BMI and smoking history, and has no significant relationship with age, cholesterol, triglyceride, blood glucose There was a significant positive correlation between LDL and syntax score, but a significant negative correlation between LDL and HDL.(6)Pearson correlation analysis of blood Hcy level and different variables in patients with H-type hypertension complicated with acute coronary syndrome was significantly correlated with high density lipoprotein level, age and SYNTAX final score, but not significantly correlated with BMI level and smoking age,but significantly positively correlated with triglyceride and cholesterol levels. And the level of low density lipoprotein was significantly negative correlation. Conclusion:Hcy is an independent risk factor for coronary heart disease, and the change of Hcy level is significantly positively correlated with the change of SYNTAS score, suggesting that plasma Hcy is correlated with the degree of coronary artery disease in patients with H-type hypertension complicated with ACS.Therefore, dynamic monitoring of changes in serum Hcy level is helpful to understand the course development of coronary heart disease, reflect the severity of cononary artery disease, and provide important basis for early prevention, early diagnosis and treatment and long-term pronosis of the disease.
