| ObjectiveTo investigate the effects of functional exercise program based on 5A model on the maturation time of arteriovenous fistula,self-management ability after arteriovenous fistula,daily upper limb mobility ability and quality of life in patients with chronic renal failure.MethodsPatients with chronic renal failure who were admitted to the department of nephrology of a third-class A hospital Autonomous Region from January 2020 to December 2020 and met the inclusion and exclusion criteria for arteriovenous internal fistula were selected.According to the random number table method,the patients were divided into two groups.The control group received routine perioperative nursing care of internal arteriovenous fistula,and the intervention group received postoperative functional exercise based on 5A mode on the basis of routine nursing.General data on preintervention evaluation of patients;Patients’ daily upper limb mobility was evaluated before intervention,4 weeks after intervention,8 weeks after intervention and 12 weeks after intervention.Patients’ self-management and quality of life were evaluated before and after intervention,and the maturation time of arteriovenous internal fistula was evaluated.ResultsA total of 86 patients were screened in this study,43 in the control group and 43 in the intervention group.Baselines are comparable.SPSS22.0 was used for statistical analysis of the data,and the specific results were as follows:1.The mature time of the intervention group was shorter than that of the control group(P < 0.001),and the blood flow of the first hemodialysis pump and the dialysis after 4 weeks were higher than that of the control group(P < 0.001).There were significant differences between the two groups in the first use of AVF natural blood flow,head vein diameter and distance to skin depth(P < 0.001).2.The results before intervention showed that there was no significant difference in each dimension of AVF self-management between the two groups(P > 0.05).After 12 weeks,the scores of AVF self-management in the intervention group were higher than those in the control group(P< 0.05).3.After 4,8 and 12 weeks of intervention,the scores of upper limb function in intervention group were higher than that in control group(P < 0.05).Multivariate analysis of variance(ANOVA)of upper limb daily activities scores at different intervention time points in intervention group and Control Group showed that there was interaction effect between group and time(P < 0.001).UNIVARIATE repeated measures design variance analysis showed that the scores of both groups increased after intervention,but the intervention group rose first and then slowly,the control group rose first and then quickly,the intervention group and the control group had the same scores at baseline,after 4,8 and 12 weeks,the scores of the intervention group were higher than those of the control group.4.After 12 weeks of intervention,the quality of life in the intervention group was higher than that in the control group(P< 0.001).ConclusionIn this study,through the application of 5A mode in the nursing of functional exercise after arteriovenous fistula operation for patients with chronic renal failure,the cognitive level of patients with functional exercise after arteriovenous fistula operation was improved,and the self-management ability and maintenance awareness of patients with AVF were improved.In this study,through the application of functional exercise program based on 5A mode,the maturation time of arteriovenous internal fistula was shortened,the incidence of AVF complications was reduced,the daily activities of the upper limbs on the fistula side of the patients were improved,and the quality of life of the patients was further improved. |
