| Objective:This study is to study the resting state function MRI(rs-f MRI)in patients with Alzheimer’s disease(AD)in the early stage of clinical subjective cognitive decline(SCD),and to explore the potential imaging markers of patients with SCD,and try to provide the basis for early recognition and early intervention of AD.Methods:From February 2019 to March 2020,22 patients with SCD,18 patients with amnestic mild cognitive impairment(a MCI)and 23 healthy control recruited from the community were selected. All patients were assessed with resting state MRI and cognitive function scale. The resting state analysis method based on amplitude of low frequency fluctuation(ALFF)and resting state functional connectivity(RSFC)was used to study the differences of brain function indexes among the three groups;meanwhile, the correlation analysis was conducted between the significantly changed ALFF and RSFC values and the scores of cognitive function scale.Results:1.Cognitive function: compared with HC group,SCD group had a significant decrease in digit span Digit span forward test, while other cognitive functions had no significant decrease;compared with HC and SCD groups,a MCI group had a significant decrease in multiple cognitive functions with memory function as the main function.2.rs-f MRI:(1)ALFF in local brain area: compared with HC group,ALFF in right lingual gyrus area of SCD group increased significantly,ALFF in right thalamus area decreased significantly,ALFF in right thalamus area of a MCI patients decreased significantly;compared with SCD group,ALFF in left superior cerebellar lobe area of a MCI patients increased significantly. There was no correlation between ALFF and cognitive score in SCD and a MCI patients.(2)RSFC of different brain regions(posterior cingulate gyrus as seed point): compared with HC group, the functional connectivity of SCD patients in left inferior temporal gyrus, left angular gyrus, left middle frontal gyrus and part of right inferior cerebellar lobe was enhanced, while that of a MCI patients in left middle frontal gyrus, left temporal pole, superior temporal gyrus and part of cerebellum was enhanced, the functional connectivity in some areas of the upper lobe of the cerebellum was enhanced in patients with a MCI.In SCD patients, the intensity of functional connectivity was negatively correlated with the CFT delay recall scores of R-O complex figure test;in a MCI patients, the intensity of functional connectivity was positively correlated with the right scores of Stroop Colour word test. Conclusion:In SCD patients, the ALFF value of local brain regions changed, and the functional connectivity of brain regions changed, which was related to some cognitive functions. It is suggested that rs-f MRI can be used as a potential imaging marker in patients with SCD. |
