| Objective1.To study the symptom burden and negative emotions of prostate cancer patients caused by androgen deprivation therapy(ADT).2.To explores the effects of diet combined with exercise on the metabolic syndrome, quality of life, fatigue, anxiety and depression of prostate cancer patients with ADT,provide guidance for clinical nursing.MethodThe first part : A prospective study was adopted to select 170 prostate cancer patients in our hospital who underwent ADT treatment for 6 consecutive months from January 2017 to July 2019.Blood pressure, blood sugar, blood lipids, waist circumference, and bone density before treatment and 6 months after treatment Change in value at the same time, the revised version of the expanded prostate cancer index composite for clinical practice(EPIC-CP)and the Revised piper fatigue scale(RPFS),Self-rating depression scale(SDS)and Self-rating anxiety scale(SAS)to assess the quality of life, fatigue, depression and anxiety of patients before and after ADT treatment. The second part: A prospective, randomized controlled method was used to divide 95 patients with prostate cancer ADT in our hospital from August 2019 to December 2020 into 46 intervention groups and 49 control groups. The control group was given routine care measures, and the intervention group was based on routine care.Carry out diet(Mediterranean diet combined with Chinese eating habits)combined with exercise(medium-intensity aerobic exercise and resistance exercise)intervention. The patient’s quality of life, fatigue, anxiety and depression were evaluated 1 day before ADT treatment and 12 weeks after intervention.ResultsThe first part: The osteoporosis, metabolic syndrome,cancer-related fatigue, anxiety, and depression in the 6 months after ADT treatment were higher than those before ADT treatment(p<0.05).Among the five dimensions of the EPIC-CP score, the sexual function and hormonal symptoms of patients with 6 months after ADT treatment were significantly higher than those before ADT treatment(p<0.05).There were not significant differences in urinary incontinence, urinary tract stimulation, and bowel function between patients before and after intervention(p>0.05).The second part: There were not significant differences in the age, education level, marital status, work status, per capita monthly income, medical expenses, serum PSA,body mass index(BMI),pre-intervention serum albumin, hemoglobin before intervention between the two groups,Gleason score, and whether radical surgery/radiotherapy were not statistically significant(p>0.05).Before the intervention, the difference in quality of life, metabolic syndrome,cancer-related fatigue, anxiety state and depression state between the two groups of patients was not statistically significant(p>0.05).After 12 weeks of intervention, there was no significant difference in the quality of life and metabolic syndrome between the two groups of patients(p>0.05).The cancer-related fatigue, depression and anxiety in the intervention group were better than those in the control group(p<0.05).ConclusionADT increases the occurrence of hormonal symptoms,cancer-related fatigue, anxiety and depression in the dimensions of osteoporosis, metabolic syndrome, and reduces the neutral function of the quality of life dimensions.Diet combined with exercise intervention can effectively reduce the fatigue of patients.Anxiety and depression. |
