| Objective To analyze the effect of sacrificing the posterior cruciate ligament(PCL)on the early postoperative outcome of cruciate retaining(CR)highly congruent rotating platform TKA.Methods From September 2018 to September 2019,88 cases of total knee arthroplasty(TKA)with CR highly congruent rotating platform prosthesis were retrospectively analyzed(There were 5 cases of rheumatoid arthritis,80 cases of osteoarthritis and 3cases of traumatic arthritis).According to the tension of posterior cruciate ligament,they were divided into sacrifice group(24 cases,27.3%)and retention group(64 cases,72.7%).Preoperative and postoperative The Hospital for Special Surgery(HSS)score,range of motion(ROM),and the general data were compared between the two groups.In addition,postoperative infection,prosthesis loosening,bearing dislocation and other complications were also compared.Results All patients were followed up for 7 ～ 20 months(mean 14.14±3.52)mo nths.The patients in the sacrifice group were followed up for 9～20 months(mea n 15.4±2.90)months,and 7～19 months(mean 13.7±3.64)months in the retention group。 There was no significant difference in Age(68.96vs69.11),Weight(65.00vs65.44),BMI(25.32vs25.51),preoperative HSS score and ROM between the two gr oups(P > 0.05).At the last follow-up,HSS score of the two groups were better than those before operation [the sacrifice group(54.75 ±6.54)vs(91.20 ±4.38),the retention group(54.98 ±6.52)vs(90.53 ±4.27)(P < 0.05)],and the ROM of t he two groups were better than those before operation[ the sacrifice group(96.58±8.72)vs(124.46±6.69),the retention group(95.56±8.35)vs(124.53±7.48)(P < 0.05)].However,there was no significant difference between the two groups(P >0.05).Moreover,there were no complications such as infection,loosening of pro sthesis and bearing dislocation in all cases.Conclusion In CR highly congruent rotating platform TKA with or without tension of the PCL can achieve satisfactory outcomes.Tension-free PCL do not cause joint instability. |
