| Objective: In this experiment,Su He Xiang essential oil aroma inhalation therapy was used to intervene patients with post-stroke cognitive dysfunction(PSCI)through the Mini-mental State Examination(MMSE),Montreal Cognitive Assessment(Mo CA)score,and serum brain-derived neurotrophic factor(BDNF)and serum homocysteine(Hcy)content comprehensively evaluate the improvement of subjects’ cognitive function before and after intervention.providing better nursing intervention measures for clinical PSCI patients,and a feasible basis for the transformation of the ward or home environment.Method: A total of 60 PSCI patients in the Second Hospital of Hebei Medical University,were enrolled in this study during October 2019 to December 2020.Inclusion criteria:(1)All patients were diagnosed according to the diagnostic criteria of the Fourth Academic Conference of National Cerebral Vascular Disease,using head CT and(or)MRI.(2)Mo CA and MMSE scores were recorded 6 months after the stroke,and Mo CA and MMSE scores were abnormal.(3)they had normal eyesight,earing and the sense of smell.(4)be able to communicate normally.Exclusion criteria:(1)Before the onset of ischemic stroke,the subjects had cognitive impairment,or cognitive impairment caused by other diseases,such as mental history,brain trauma,etc.;(2)severe alcoholism and smoking;(3)use of sedatives;(4)severe organic diseases,or cognitive impairment caused by other diseases;(5)participating in or participating in other clinical research.General information of patients was collected,including age,gender,education level,history of smoking and drinking,history of previous diseases,drug use,olfactory nerve function,etc.the included patients were scored with MMSE,Mo CA,serum Hcy and BDNF.Intervention measures: 60 patients with PSCI were randomly divided into the control group and the intervention group,30 cases in each group.Both groups were given routine treatment and nursing to keep the appropriate temperature and humidity,and keep the intervention environment clean and odorless.The control group was given pure water to sniff,and the intervention group was mixed with pure 100 ml of 4ml in the pure water to give the participants a fragrant environment.The intervention time was9:00-11:00 and 19:00-21:00 every day,lasting for 30 minutes each time for 8weeks.The serum levels of BDNF,Hcy,MMSE and MOCA were compared before and 8 weeks after intervention.Data were analyzed using SPSS21.0and measurement data were expressed as mean ± standard deviation(SD).Differences between two groups were compared by independent samplet-tests and paired t-tests were used to compare the data before and after the intervention within each group.P<?0.05 was considered significant statistical differences.Results: There was no significant difference in the Mo CA、MMSE scores between the two groups before treatment(P>0.05).After 8 weeks of intervention,the scores of MMSE and Mo CA in the intervention group(25.33±2.881)and(19.27±3.69),were significantly higher than those in the control group(21.33±2.32)and(15.43 ±2.93)(P<0.01).The score before and after intervention in the intervention group was significantly higher than that before intervention(22.13±2.93)and(16.60±3.91),respectively,and the difference was statistically significant(P<0.01).There was no significant difference in serum HCY level between the two groups before intervention(P>0.05).After 8weeks of intervention,the serum Hcy level in the intervention group(8.67±1.32)μmol/L was significantly lower than that in the control group(11.09±1.32)μmol/L(P<0.01).The serum Hcy level before and after intervention in the intervention group was significantly lower than that before intervention(12.73 ± 1.90)μ mol/L(P<0.01).After 8 weeks of intervention,the difference value of serum BDNF in the intervention group was(10.70±6.72),which was significantly higher than that in the control group(-0.98 ±6.79)ng/ml(P<0.01).The serum BDNF level before and after intervention in the intervention group(37.41±4.38)was significantly higher than that before intervention(26.71±4.58)ng/ml(P<0.01).The serum BDNF level before and after intervention in the intervention group(37.41±4.38)was significantly higher than that before intervention(26.71±4.58)ng/ml(P<0.01).Conclusions:1.Su He Xiang essential oil can improve cognitive function in patients with cognitive dysfunction after stroke.2.The content of BDNF increased and Hcy decreased in patients with cognitive impairment after stroke.3.The improvement of cognitive function may be caused by changes in serum BDNF and Hcy levels after the smell of essential oil. |
