Objective:Depression is one of the common chronic mental disease.Antidepressant drugs are the first line of treatment for depression,but some patients do not respond to drug treatment,and there are side effects.In recent years,non-drug treatment of depression has attracted more and more attention.Therefore,the purpose of this study was to investigate the efficacy and adverse reactions of transcutaneous cranio-auricular electrical stimulation in the treatment of depression,and to compare it with the antidepressant escitalopram oxalate,so as to provide a new treatment method and idea for the clinical treatment of depression.Methods:Subjects were recruited from patients with depression who visited the Mental Health Department of the First Hospital of Hebei Medical University from September 2019 to December 2020.A total of 72 patients were screened in the outpatient department,and 67 subjects were eventually included in the study and 60 subjects were included in the analysis.SAS software was used to generate a random number table.The patients were randomly divided into two groups: transcutaneous cranio-auricular point electrical stimulation group(30cases)and escitalopram oxalate group(30 cases).The treatment period was 4weeks.Before treatment,mood assessment was performed by Montgomery Depression Rating Scale(MADRS),Hamilton Depression Rating Scale(HAMD-17items)and Hamilton Anxiety Rating Scale(HAMA)for 2 and 4weeks respectively,and adverse reactions were recorded(ADR Record).Pittsburgh Sleep Quality Index(PSQI)and Quality of Life Questionnaire(SF-36)were used to evaluate sleep quality and quality of life before treatment and 4 weeks after treatment,respectively.SPSS26.0 was used for statistical analysis of the data after treatment.Repeated analysis of variance or t test was used for measurement data,and rank sum test was used for data that did not conform to normal distribution.Counting data were tested by chi-square test.Univariate and multivariate linear regression were performed for the factors affecting HAMD score.The relationship between HAMD score difference and age,course of disease and frequency of attack between the two groups before and after treatment was analyzed by Pearson correlation.P<0.05 was considered statistically significant.Patients with incomplete data were not included in the statistical analysis.Results:A total of 72 patients were screened,including 1 patient who did not receive treatment and 4 patients who could not complete the scale assessment due to living in other places,and 67 patients were eventually included.1、Compared with the escitalopram oxalate group,there were no significant differences in gender,age,ethnicity,marital status,education level,occupation,monthly family income,history of alcohol consumption,smoking history,and baseline scale scores between the transcutaneous cranio-aural acupoint stimulation group and the escitalopram oxalate group.2、The scores of MARDS,HAMD and HAMA in the study group and the control group decreased significantly after treatment(P=0.000),but there was no statistically significant difference between the study group and the control group(P>0.05).The response rate of the two groups was 50% in the study group and 43.3% in the control group,and the difference was not statistically significant(P>0.05).3、Compared with the control group,the difference of PSQI total score(P=0.001),sleep efficiency factor(IV)(P=0.001),daytime dysfunction factor(VII)(P=0.016)in the study group before and after treatment was greater,and the difference was statistically significant(P<0.05).4、Compared with the control group,the energy(VT)dimension of SF-36 in the study group was significantly increased after treatment(P=0.025),and the difference was statistically significant(P<0.05).5、The incidence of adverse reactions in the study group was 6.7%,and the incidence of adverse reactions in the control group was 43.3%.The difference in the incidence of adverse reactions between the two groups was statistically significant(P>0.05)Conclusions:1.The overall efficacy of TECAS was similar to that of escitalopram oxalate in the treatment of depression.2.Increase patients’ energy,improve patients’ subjective sleep quality,especially improve sleep efficiency and sleep efficiency TECAS had a better effect than escitalopram oxalate in terms of daytime dysfunction.3.The percutaneous cranio-auricular electrical stimulation has less adverse reactions and high safety.
