Object: To explore the diagnosis value of liver biopsy for unknown cause liver disease,and analyzes the security of the liver biopsy.Methods: Patients admitted for liver diseases from January 1,2000 to January 1,2018 were collected in the medical record room of the First Affiliated Hospital of Shihezi University.Patients who could not be diagnosed with liver diseases of unknown cause or liver space occupying lesions after conventional non-invasive examination,clinical symptoms and signs combined with the patient’s medical history were screened for liver puncture for diagnosis or treatment of liver diseases of unknown cause or liver space occupying lesions.At the same time,the basic information of 165 patients,such as age,gender,nationality,work,past medical history,smoking and drinking history,height and weight,were collected and treated.The results of liver function,blood analysis,blood clotting function,hepatitis A,hepatitis B and hepatitis C,anti-liver antigen antibody,autoimmune antibody,liver fibrosis,thyroid function and the data of upper abdominal color ultrasound and CT/MRI after admission were collected and summarized.The basic characteristics of the patients’ diseases were summarized,and preliminary clinical diagnosis was made according to the patient’s condition.Meanwhile,the pathological diagnosis results and complications of liver puncture were summarized,and the data of liver pathological and clinical characteristics and complications after liver puncture were analyzed and summarized.Results:(1)In this study,165 patients underwent liver biopsy for diagnosis or treatment of patients with liver biopsy in diagnosis of 115 cases,including diagnosis of 107cases(93.0%),including 20 cases(17.4%)of autoimmune liver disease,including 3 cases of cholangitis correlation disease,1 case of cholestasis liver cirrhosis,hepatitis viral hepatitis/normal 39cases(33.9%),There were 19 cases of non-viral hepatitis and 3 cases of viral hepatitis.17 cases of patients progress to liver cirrhosis,5 cases of liver cyst(4.3%).8 cases of liver abscess(7.0%),hepatic malignant tumor in 23 cases(20.0%),3 cases of non-alcoholic fatty liver(2.6%),4 cases of benign liver tumor(3.5%),2 cases of hepatic subcapsule effusion(1.7%),1 case of schistosoma cirrhosis,1 case of drug-induced hepatitis and 1 case of Gilbert syndrome(0.9%).In addition,8 patients(7.0%)could not be definitively diagnosed by liver biopsy.(2)Among the 101 patients with clear diagnosis,58 patients(54.2%)had basically consistent clinical diagnosis with pathological diagnosis,and 49 patients(45.8%)had inconsistent clinical diagnosis with pathological diagnosis.Among the patients diagnosed by liver puncture,8patients failed to be clearly diagnosed by pathological examination of liver puncture,The other 50 cases were treated with liver puncture alone and all of them were effective.(3)Of the 165 patients with liver puncture,15 patients developed complications,including 12patients(7.3%)with mild pain at the puncture site,1 patient(0.6%)with fever,1 patient(0.6%)with subcapsular hemorrhage,and 1 patient(0.6%)with chest tightness.Conclusion: Liver biopsy is of certain value in the diagnosis of liver diseases with unknown causes and liver space occupying lesions.Hepatocentesis has low probability of complications and high safety.
