Objective:To explore the effect of ear acupoint burying bean on epidural labor analgesia for urinary retention during delivery,and analyze the effects of ear acupoint burying bean on delivery outcome,urination,anxiety,depression,pain and comfort.Methods:Choice in May 2020 to January 2021 in a third rate for women’s and children’s hospital maternity ward vaginal trial production and accept the epidural labor analgesia of 240 cases of primipara as the research object,using the random number table,120 cases were randomly divided into intervention group and control group each intervention group with ear acupuncture point buries bean under routine nursing,the control group accepted routine nursing care.Eventually eliminate accept maternal vaginal midwifery and transit cesarean delivery all women into a complete general data questionnaire,general Hospital Anxiety Depression scale(HAD),Visual Analogue Score(VAS)and General Comfort Questionnaire(GCQ)were analyzed.Using SPSS25.0 software was used for statistical analysis.Measurement data were represented as((?)±S),independent sample t test was used for comparison of measurement data with normal distribution,and non-parametric rank sum test was used for variance inhomogenies.Enumeration data were expressed as constituent ratio(%).The two groups of enumeration data were compared by x~2 test.The mixed linear model was used to analyze the effects of general data on the results in the case of grouping.Results:1.The intervention group eventually into the group of 105 cases,control group eventually into the group of 103 cases,compares and analyses the general situation of two groups of maternal results show that the two groups of women in the demographic characteristics,lasting transfusion amount,eating and drinking,into the delivery room for the delivery of VAS score,etc,in contrast,there were no statistically significant difference(P>0.05),shows that two groups of comparable statistical sample.2.The effect of ear acupoint burying bean on the urination of parturient women after epidural labor analgesia.(1)A statistical analysis was made on the residual urine volume of B-ultrasound in the period of 5~6cm opening of the uterine opening,complete opening of the uterine opening and 2h postpartum of the two groups of women.The results showed that when the uterine opening was 5~6cm,the mean value of residual urine volume of B-ultrasound in the intervention group was(88.76±81.11)ml,and that of the control group was(127.55±108.98)ml.The residual urine volume of B-ultrasound in the intervention group was significantly lower than that of the control group(P<0.05).At the time of full opening of the uterine mouth,the residual urine volume of B-ultrasound in the intervention group was(100.10±114.60)ml,and that of the control group was(95.92±86.53)ml.There was no significant difference between the two groups(P>0.05).At 2h postpartum,the mean residual urine volume of B-ultrasound in the intervention group was(81.67±108.81)ml,while that of the control group was(99.42±112.49)ml,and there was no significant difference between the two groups(P > 0.05).(2)The results of mixed linear model analysis showed that in the fixed effect,there was a statistical difference in the influence of age on the residual urine volume of B-ultrasound when the uterine orifice was 5~6cm open(P<0.05).In the random effect,there was statistically significant difference in the residual urine volume of B-ultrasound when the uterine opening was 5~6cm(P< 0.05).There was no statistical significance in the effect of intercept +infusion volume+ water intake during labor on the residual urine volume of B-ultrasound when the uterine oritis was 5~6cm open(P< 0.05).(3)The scores of the first urination status,abdominal symptoms,urination time and total scores of the two groups were comprehensively compared.The results showed that the score of the first urination status in the intervention group was(0.50±0.99)points,while that in the control group was(1.13±0.76)points,which was significantly lower in the intervention group than that in the control group,with statistical significance(P<0.01).The abdominal symptom score of the intervention group was(0.30±0.65)points,and that of the control group was(0.70±0.95)points.The abdominal symptom score of the intervention group was significantly lower than that of the control group,the difference was statistically significant(P<0.01).The first urination time score of the intervention group was(0.50±0.93)points,while that of the control group was(0.98±1.12)points,and the difference between the two groups was statistically significant(P<0.01).The total score of first urination in the intervention group was(1.60±2.0)points,and that in the control group was(2.81±2.41)points,which was significantly lower in the intervention group than that in the control group,and the difference between the two groups was statistically significant(P<0.01).(4)Analysis results of mixed linear model showed that under the premise of grouping,only neonatal weight had influence on maternal urination status,abdominal symptoms and urination time score,while other general information had no influence on maternal urination status,abdominal symptoms and urination time score(P>0.05).In the random effect,the residual had an impact on the maternal urination status,abdominal symptoms and urination time score(P<0.05),so it was considered that the individual differences between the two groups of intervention subjects had an impact on the maternal urination status,abdominal symptoms and urination time score.(5)Comparison of the time between the first postpartum urination and delivery time between the two groups: the time between the first postpartum urination and delivery time in the intervention group was(231.37±47.71)minutes,while that in the control group was(287.16±142.61)minutes.The time in the intervention group was shorter than that in the control group,the difference was statistically significant(P <0.05).3.The effect of ear acupoint burying bean on the pain of epidural labor analgesia.The VAS scores of the two groups when the uterine opening was 5~6cm and the uterine opening was full were studied.The results showed that the average VAS scores of the intervention group and the control group were(6.04±2.61)points and(7.0±7.13)points respectively,and the difference between the two groups was statistically significant(P<0.05).There was no significant difference in VAS scores between the two groups at the time of uterine opening(P>0.05).4.Influence of ear acupoint burying bean on comfort level of epidural labor analgesia.(1)The comfort level of the two groups was compared.The total comfort level of the intervention group was(83.11±7.86)points,including physiological score of(13.66±2.47)points,psychological score of(28.87±3.07)points,social and cultural score of(18.57±2.02)points,and environmental dimension of(21.48±2.50)points.Control comfort score(80.88-9.86)points,including the physiological score for(11.75±2.73),psychological spirit score(27.75±4.22)scores,social culture is(18.70±2.69)points,divided into environmental dimension(19.94±3.05),and the intervention group comfort,physiological score and total score of psychological mental scores were significantly higher than that of control group(P<0.05),two groups of social culture and environment score had no significant difference(P>0.05).(2)Analysis method of mixed linear model: in the fixed effect,occupation had a statistical difference in the influence of comfort rating(P<0.05),while other general information had no statistical significance in the influence of comfort rating(P>0.05);In the random effect,the residual had a statistically significant effect on the comfort score(P<0.05).There was no statistical significance in the effect of interception,infusion volume and water intake during labor on comfort score.5.Influence of ear acupoint burying bean on maternal mood of epidural labor analgesia.The results showed that there were no significant differences in anxiety scores,depression scores and HAD scores between the intervention group and the control group(P>0.05).6.Effect of ear acupoint burying bean on childbirth outcome of epidural labor analgesia.(1)The results showed that there was no statistically significant difference in the third stage of labor between the two groups(P>0.05),and there were statistically significant differences in the rest time(P<0.01).(2)Through the analysis and statistics of the fever during labor in the intervention group and the control group,there was no statistical significance in the fever between the two groups(P>0.05).In addition,the neonatal hospitalization and postpartum 2h blood loss in the intervention group and the control group were compared and analyzed,and the results showed that there was no statistical significance in the postpartum 2h blood loss,neonatal hospitalization and neonatal asphyxia between the two groups(P > 0.05).Conclusion:Ear acupoint burying bean can improve urinary retention for epidural labor analgesia during early active(the uterine opening 5~6cm),and obviously shorten the delivery time,can effectively promote the process of postpartum urine,first voiding degree,abdominal symptoms and improve maternal urination wait for conditions such as time,at the same time can effectively improve maternal physiological,mental,spiritual comfort.Therefore,ear acupoint burying bean is a safe and effective traditional Chinese medicine treatment method to prevent and cure epidural delivery and relieve the maternal urinary retention and improve the postpartum comfort.It has clinical application value,and can be popularized and applied in future clinical practice in combination with clinical practice.
