Objective:This article aims to study the clinical characteristics and survival prognosis of double primary cancers in the stomach and colorectum. Methods:This article collected the clinical case data of 38 patients with gastric and colorectal double primary cancers who were admitted to the Fourth Hospital of Hebei Medical University from January 2015 to October 2020,and 76 Patients with single primary stomach cancer who were selected by random sampling served as controls. There were Performing retrospectively research on the patient’s general condition, test data, treatment, tumor location, pathological characteristics and survival prognosis.and the data were statistically analyzed by using SPSS26.0.Results:Among 38 patients with double primary cancers of the stomach and colorectum, having 30 males and 8 females. The ratio of male to female was 3.75:1,and the average age of onset was 63.8±9.2 years.Compared with patients who were diagnosed single primary gastric cancer, patients with double primary cancers of the stomach and colorectum accounted for a higher proportion in abnormal tumor markers(P=0.003),most of whom had a history of drinking(P=0.039),anemia(P=0.009),and Hypoproteinemia(P=0.024).However,there was no significant difference in smoking history and family history between the two groups. The rate of radical surgery was 44.7%in patients with double primary cancers of the stomach and colorectal, which was lower than that of the single primary cancer group(P=0.000).In the double primary cancer group with the single primary cancer group, the gastric cancer lesions were mostly located in the gastric antrum. In patients with double primary cancers of the stomach and colorectum, more colorectal cancers were located in the rectum, accounting for 52.6%.There was no significant difference in the pathological staging, pathological type and differentiation degree of gastric cancer between the two groups. And also, there was no statistical difference in overall survival(OS)between the double primary cancer group and the single primary cancer group(P=0.461).In this study, there were 22 cases of metachronous gastric and colorectal double primary cancers, accounting for 57.9%.And 16 cases of simultaneous gastric and colorectal double primary cancers, accounting for 42.1%.The ratio of the two groups is 1.38:1.Comparing patients with simultaneous and metachronous gastric and colorectal double primary cancers,it was showed that the gastric cancer lesions of the two mostly occurred in the gastric antrum, and the colorectal cancer lesions were located at different locations(P=0.037).The metachronous double primary cancer group was more common in the left colon. However, the colorectal cancer lesions in the left and right colons were not significantly different in the simultaneous double primary cancer group.Compared with patients in the simultaneous double primary cancer group, patients in the metachronous double primary cancer group had earlier gastric cancer stages(P=0.001)and a better prognosis(P=0.000).Conclusion:1.Stomach and colorectal double primary cancers are more common in elderly male patients, with anemia and hypoalbuminemia.2.Patients who are diagnose gastric cancer should be paid attention to colorectal cancer screening, especially for rectal cancer,to avoid missed diagnosis.3.There is no significant difference between the prognosis of patients with double primary gastric cancer and colorectal cancer and patients with single primary gastric cancer. For these patients,we should do a good job in education and establish diagnosis and treatment confidence to avoid delays and loss of treatment opportunities.4.There is a significant difference in gastric cancer staging between simultaneous gastric and colorectal double primary cancers and metachronous gastric and colorectal double primary cancers, and patients with metachronous double primary cancers generally have earlier gastric cancer staging and better prognosis.5.The shorter the time interval between the onset of gastric cancer and colorectal cancer, the worse the prognosis.
