| Objective(1)According to the relevant ancient literature and large sample in vivo meas-urement data to standardize the chest bone value,to achieve the accurate location of chest dangerous acupoints.(2)Display the stereo location of chest dangerous acupoints and three-dim-ensional visualization of acupuncture path based on HRCT technology.(3)Multi-level and multi-angle display the spatial relationship between chest dangerous acupoints and human anatomical structure,and explore the danger and safe depth of acupuncture during direct acupuncture,so as to provide reference basis for acupuncture clinic and teaching.Methods Experiment one:(1)Using the Chinese Medical Code as the main retrieval tool,and with the help of the database such as Superstar data Library,traditional Chinese Medicine Ancient Books,traditional Chinese Medicine Resources Network and so on,we comprehensive-ly search the ancient documents related to bony-scale.(2)According to the inclusion and exclusion criteria,the provisions related to the method of bone-scale positioning are manually selected,and the database of "Ancient documents of bone-scale" is established by using Office Excel.(3)Summarize the bone size that have not changed in ancient and modern times and that have changed in the chest.(4)The bone of 200 volunteers who had not changed from ancient to modern and the changes in chest were measured.Experiment two:(1)21 volunteers were selected according to the inclusion and exclusion criteria.(2)Prepare before scanning:a.Record the name of the volunteers,measure their height and weight,and calculate the BMI value;b.Remove foreign bodies,refer to the national standar-d "Name and Positioning of Acupoints",and location 22 acupoints in the chest according to "8cun between the inner edges of two areola","9 cun from the superior sternal fossa(Tiantu point)to the midpoint of sword-chest combination(Qigu)",and "from the midpoint of sword-c hest combination(Qigu)to 9 cun in the navel";c.The licorice tablet is fixed at the marked acupoint as a marker of the acupoint;d.Scanning position:supine position,head tilted to the right to expose the sternocleidomastoid muscle,arms slightly extended,palms upward in an anatomical position,the body in the middle of the bed,so that the median sagittal plane of the chest and the midline of the bed are on the same plane.(3)Scan:a.Using Philips 256-layer extreme speed CT machine for spiral thin-layer scanning;b.Scanning range: continuous 2cm scanning from the apex of the lung to the posterior costal diaphragmatic angle;c.Scan slice thickness 0.625 mm,increment:-0.321mm;d.Matrix: 1024×1024;e.Photo window:mediastinal window:window width 500 HU,window level:30HU;Lung window:1500HU wide,window lev-el:-700HU;f.Positioning image scanning field(FOV):350mm.(4)Data storage:the imaging information obtained is stored in the DVD+ R disk in DICOM format.(5)Data processing:the HRCT image in DICOM format is directly imported into the medical image processing software by using the image import tool,and the three-dimensional models of skin,chest,bronchus,lung,lung,heart,fat,muscle and acupoints are established by using "model processing","image segmentation" and "3D modeling".(6)Using 3D medical image reading software,a descriptive three-dimensional visualization platform of chest dangerous acupoints is establish-ed.(7)The needle penetration depth of chest dangerous acupoints is measured based on the three-dimensional visualization platform of chest dangerous acupoints,and the dangerous depth×75% is taken as the safe depth.Results Experiment one:1.Study of ancient literature:(1)There were 27 ancient books on bone-scale,most of which were related to acupuncture,massage and moxibustion.(2)There are 6 bones unchanged in ancient and modern times,including 2 in the head and face and 4 in the lower limb.(3)The bone of the chest has changed.2.Research on large sample Measurement:(1)In the volunteers with normal,overweight and obese BMI,the bone measurement values of "greater trochanter to popliteal transverse stria(flat patellar tip)" and "popliteal transverse stria(flat patellar tip)to lateral malleolus tip" were correlated with BMI(P < 0.05);The bone measurements values of "between two nipples" and "between the medial edge of areola" were significantly correlated with BMI(P < 0.001);Th-ere was no correlation between the values of bone and BMI in other parts of the body(P >0.05).(2)The bone of the chest was determined according to "9 cun from Tiantu point to the Qigu","9 cun from Qigu to the navel","9.5 cun between the two nipples" and "8 cun between the medial edge of the areola".Experiment two:(1)Based on the HRCT image information,the medical image processing software can be used to realize the three-dimensional expression of the structure of human acupoints,tissues and organs,and can directly observe the distribution of the internal anatomic-al structure of the human body and the individual differences of the anatomical structure.(2)It can show the spatial relationship between chest dangerous acupoints and surrounding tissues and organs,and realize the visualization of acupuncture path.(3)Positive view:the risk degree of Ren Meridian,Foot Shaoyin Kidney Meridian,foot Yangming stomach meridian,foot Jueyin liver meridian and hand Jueyin pericardium meridian through chest acupoints is relatively high,while that of foot Taiyin spleen meridians through chest acupoints is relatively small.han-d Taiyin lung meridian chest acupoints are not dangerous.(4)Of the 22 chest acupoints,15 are bound to be dangerous.(5)When directly puncturing the three acupoints of Shufu point,Zhongfu point and Yunmen point,there is no danger.(6)The safe depths for direct puncture of the Tiantu point,Yuzhong point,Shencang point,Lingxu point,Shenfeng point,Bulang point,Qis-he point,Wuyi point,Yingchuang point,Rugen point,Qimen point,Tianchi point,Xiongxiang point,and Shidou point are 24.05 mm,16.57 mm,18.06 mm,20.44 mm,20.18 mm,20.32 mm,24.71 mm,22.88 mm,23.38 mm,16.39 mm,15.98 mm,24.27 mm,32.73 mm,26.71 mm,17.93 m.(7)a.There was no correlation between the acupuncture risk depth of Tiantu point,Bulang point,Qishe point,Tianxi point,Shidou point with BMI;b.There was a correlation between the dangerous depth of acupuncture and BMI during direct needling in Yuzhong point,Rugen point,Qimen point,Tianchi point.The regression equations related to BMI are: y(28)0.262(10)1.084 x,y(28)-3.724(10)1.225 cun between two nipples" and "8 cun between the medial edge of areola".2.The application of HRCT technology and image processing technology to acupuncture and moxibustion can realize the three-dimensional expression of the structure of human acupo-ints,tissues and organs,and observe the spatial relationship between chest dangerous acupoi-nts and human anatomical structure and acupuncture path from multi-level and multi-angle. |
