Research On "Internet +" Home Nursing Management Model For The Elderly Based On Family Health Responsibility Person

Objective1.To understand the current situation of family health care resources,the use of "Internet+ Medical treatment" and the level of health behavior self-efficacy of the elderly in the community,so as to provide basic data for the construction of community nursing management model for the elderly.2.To establish an "Internet +" family nursing management model for the elderly based on family health responsibility person,so as to provide a tool for community nursing management of the elderly.Method1.Questionnaire: using the self-designed questionnaire on the use status and health behavior self-efficacy of "Internet + Medical Care" for the Community elderly,a questionnaire survey was conducted on the personal data,family information,use and demand of "Internet +Medical Care" and the level of health behavior self-efficacy of 336 community elderly people in the jurisdiction of two community health service institutions in Yinchuan.2.Expert consultation method: based on reference to domestic and foreign literature,combined with the results of previous questionnaire survey,and supported by relevant policies,the embryonic form of "Internet +" family nursing management model for the elderly based on family health responsibility person was constructed.Using Delphi method,after two rounds of expert consultation,the family nursing management model of "Internet +" for the elderly based on family health responsibility person was determined.Result1.The results of the survey on family information of 336 elderly people in the community showed that the average total score of family function was 8.87 ±1.84,and most of the subjects(88.7%)had good family function.The main type of family structure was "nuclear family(44.9%)",the family power structure was mainly "traditional authority type(63.1%)",and the living condition was mainly "those who live only with spouses(67.6%)".But when the elderly need to be taken care of,they are mainly "children(59.5%)".2.Among the elderly in the community,32.4% were using smartphones,9.5% have heard of "Internet + Medical" service.The most frequently used "Internet + Medical" service was "outpatient appointment registration".The main reason for not using this kind of service was "unable to operate(complicated operation)".3.The scores of health behavior self-efficacy of the elderly in the community was 76.07±31.98,and the scores of each dimension was 19.67 ±8.71 for exercise,19.64 ±8.67 for health responsibility,18.74 ±8.06 for mental well-being and 17.96 ±7.53 for nutrition.The results of multiple linear regression analysis showed that gender and monthly income could explain23.8% of the total variation of health behavior self-efficacy of the elderly in the community.4.Through two rounds of expert consultation,we finally determined the "Internet +" family nursing management model for the elderly based on family health responsibility person.It includes five parts: service object,management team,selection of family health responsibility person and training content,implementation method and management content.After two rounds of expert consultation,the positive coefficients of experts were 100%,the authority coefficient was 0.87,the coordination coefficient was 0.226,the coefficient of variation of the feasibility of each index was 0.07,and the expert opinions were more concentrated.Conclusion1.The utilization rate of "Internet + Medical Care" for the elderly in Yinchuan community was low,but they have adequate the family nursing resources,and most of the elderly were willing to be remotely managed by community medical staff by themselves or with the assistance of their families.2.The level of health behavior self-efficacy of the elderly in the community of Yinchuan was low,and they lack the ability of nutrition.The main factors affecting health behavior self-efficacy were gender and monthly income.3.The family nursing management model of "Internet +" for the elderly based on family health responsibility person was constructed.It includes five parts: "service object","management team","selection of family health responsibility person and training content","implementation mode" and "management content" and some specific content items.Through the analysis and test of the data of the consulted experts,the results show that the credibility was high.4.The model constructed in this study can provide a tool for community nursing management of the elderly and provide theoretical basis for related research.
