Objective: This study intends to analyze the risk factors of progressive stroke in patients with cerebral ischemic stroke providing theoretical basis for the prevention of progressive stroke.Methods: A total of 266 patients with cerebral ischemic stroke from 2018 to 2019 were studied by retrospective analysis.All the patients were from the Department of Neurology of the first people’s Hospital of Yinchuan City,the second affiliated Hospital of Ningxia Medical University.These patients with acute stroke were divided into stroke in progression(SIP)group and not stroke in progression(NSIP)group according to NIHSS score.NIHSS score selected five time points,including admission,disease change,3 days before admission,7days after admission and discharge,to evaluate the clinical status of patients at these five time points.A total of 40 patients in SIP group met the diagnostic criteria and 226 patients in NSIP group were divided into two groups.The basic personal information of the two groups(gender,age,history of smoking,hypertension,diabetes,etc.)and vital signs of admission(systolic blood pressure,diastolic blood pressure,pulse pressure difference,etc.)and laboratory test results(blood routine,serum uric acid,triglyceride,low high density lipoprotein,etc)and indicators that reflect the body’s blood coagulation function(fibrinogen,D-dimer)were analyzed and compared.In addition,homocysteine,glycosylated hemoglobin,craniocerebral nuclear magnetic resonance or CT were used to determine the responsible focus.Results:1.There were a total of 266 patients with cerebral ischemic stroke,of which 40 had progressive stroke.It indicated that the incidence of the disease was 15.04%.2.there were significant differences in hypertension,diabetes and their combination between the two groups(P < 0.05).3.The comparison of admission blood pressure,systolic blood pressure and pulse pressure difference between the two groups was statistically significant(P < 0.05).4.There were significant differences in white blood cell count and high density lipoprotein between the two groups(P < 0.05).5.The results of carotid plaque between the two groups showed that there was significant difference in the detection rate of unstable plaque between the two groups(P < 0.05).6.The results of responsible focus between the two groups showed that there was significant difference in watershed infarction(P < 0.05).Conclusion: hypertension,diabetes mellitus,hypertension complicated with diabetes,increased systolic blood pressure,increased pulse pressure difference,leukocytosis,decrease of high density lipoprotein,unstable carotid plaque and watershed infarction are related to progressive stroke.
