| Objective:To explore the value of biparameter MRI radiomics in the differential diagnosis of benign and malignant prostatic hyper-b-value DWI high signal lesions. Methods:From January 2016 to August 2019,81 patients(34 benign and 47 malignant),with hyperintense b-value DWI nodule of prostate and biopsy pathological results within4 weeks were analyzed retrospectively. All the patients were scanned with two parameter MRI in transverse plane. The second edition of prostate imaging reporting and data system score of lesions according to DWI images,DWI(b=3000s/mm2)lesions were automatically delineated using Beijing Dr.Wise multimodal scientific research platform, and T2WI fat pressing and DWI(b=1000s/mm2)lesions were semi-automatically delineated using it as the standard, the image group features were extracted from all the labeled ROI,and the feature parameters with a deletion rate of more than 10%and an autocorrelation p≥0.9 were eliminated, then L1 Based method was used to reduce dimensions, and Random Forest(RF)method was used to build a diagnostic model, the diagnostic model was tested by the five-fold cross validation method. The area under the ROC curve(AUC)of the subjects was measured, and the sensitivity and specificity of the diagnostic model were calculated with the maximum approximate index as the critical value. Results:The AUC of PI-RADS v2 score was 0.653,the sensitivity and specificity were0.717 and 0.486,respectively.Correlation analysis, dimensivity reduction and RF method were used to construct the diagnostic model for the features extracted from the target lesions of 81 patients.9、10 and 9 features with original information were obtained by T2WI fat pressing and DWI b=1000s/mm2 and b=3000s/mm2,respectively,including 2common features(elongation, sphericity). The AUC of the three study sequences to distinguish benign and malignant prostate lesions in the validation set was 0.723,0.800and 0.833,respectively.The sensitivities were 0.696,0.732 and 0.821,respectively.The specificities were 0.689,0.733 and 0.756,respectively.Conclusion:the radiomics model based on biparameter MRI is valuable in differentiating benign and malignant lesions with high b-value DWI signal, and its diagnostic efficiency is higher than that of PI-RADS v2 scoring system. |
