Objective:To investigate the correlation between the neutrophil to lymphocyte ratio(NLR),red blood cell distribution width(RDW)and the level of Hypersensitivity CReactive Protein(hs CRP)within 4.5 hours of onset and the prognosis of intravenous thrombolysis in acute cerebral infarction patients using recombinant tissue plasminogen activator(rtPA)before intravenous thrombolysis.Methods: Patients with acute cerebral infarction within 4.5 h of onset who were admitted to the Department of Neurology of Shenyang First People’s Hospital from January 2019 to December 2019 were selected and collected as study subjects,and the sample size was included for a total of 120 cases.Data on patients’ general conditions such as age at onset,gender,previous history,personal history,and other medical history,white blood cell count(WBC),neutrophil number(neut),lymphocyte number(lym),red blood cell distribution width(RDW)and Hypersensitivity C-Reactive Protein(hs CRP),uric acid,lipid profile,and blood glucose were collected and collated,and neut was calculated Ratio(NLR)to lym,measured the blood pressure before thrombolysis in patients,and asked about the smoking history of the study subjects.At 3 months after onset,patients were recruited and interviewed by professionals at the follow-up center of our hospital,and they were scored and recorded according to the modified Rankin Scale(MRS),with Mrs > 2 classified as poor prognosis and Mrs ≤ 2 as good prognosis.SPSS 25.0 software was used for statistical analysis,the t-test or non parametric test was selected for baseline data,and the relationship between some indicators such as NLR,RDW and hs CRP and the prognosis of thrombolysis in acute cerebral infarction patients was selected by multiple logistic regression analysis..Results: 1.In this study,the mean age was(64.95 ± 10.21)years in the good prognosis group and(69.36 ± 9.68)years in the poor prognosis group,and the difference was statistically significant(P < 0.05).The mean NIHSS score at onset was(4.56 ± 3.05)in the good prognosis group and(11.5 ± 5.23)in the poor prognosis group,and the difference was statistically significant(P < 0.05).2.WBC,neut,lym,NLR within 4.5 hours of acute cerebral infarction onset and other indicators including hs CRP,RDW,urea nitrogen were associated with poor prognosis,with significant differences(P <0.05).3.A multivariate logistic regression analysis was used to identify neutrophil to lymphocyte ratio(NLR),red blood cell distribution width(RDW),and Hypersensitivity C-Reactive Protein(hs CRP)as independent risk factors for outcome in patients with acute cerebral infarction treated with intravenous thrombolysis with rtPA within 4.5..Conclusion: 1.Acute cerebral infarction patients with advanced age at admission and high NIHSS score at admission were more likely to have a poor recent prognosis.2.Pre thrombolysis NLR,RDW,hs CRP can be used as independent predictors of recent outcome in patients with acute cerebral infarction treated with intravenous thrombolysis with rtPA.2.Elevated NLR,RDW,and hs CRP before thrombolysis all increase the risk of developing a poor recent outcome in patients with acute cerebral infarction treated with intravenous thrombolysis with rtPA..
