Objective:The aim of this study was to investigate the effect of the modified WATIAN drinking water test combined with swallowing disorder training on the incidence of aspiration pneumonia in stroke patients with dysphagia,and to analyze the risk factors that may increase the incidence of aspiration pneumonia.Methods:The subjects were stroke patients with dysphagia who were hospitalized at the Sheng-jing Hospital of China Medical University from May 2016 to April 2018.A total of 50 patients were selected as the control group,and 50 patients were selected as the experimental group from May 2016 to the end of April 2017.The patients were selected from May 2017 to April 2018 to increase the screening of modified WATIAN drinking water test before swallowing training,according to the results of screening and feeding guidance patients,a total of 50 cases as a screening plus training group.The incidence of AP was compared between the two groups,and the influence of modified WATIAN drinking water test and ingestion instruction on the incidence of AP was analyzed by variance analysis.At the same time,80 patients with post-stroke dysphagia who were hospitalized in rehabilitation center from May2017 to December 2019 were studied and analyzed.First,the independent variables were analyzed by single factor analysis,and the factors with statistical difference were analyzed by Logistic regression analysis to get the risk factors of AP.First,the independent variables were analyzed by single factor analysis,and the factors with statistical difference were analyzed by Logistic regression analysis to get the risk factors of AP.Results:1.Add the modified WATIAN drinking water test to the swallowing disorder training.Based on the results of the test,give guidance on how to eat,where to eat,how much to drink,and how much to drink,the incidence of AP has decreased.The incidence was 38%in the control group and 16%in the experimental group.The difference was statistically significant(χ~2=6.139,P=0.013).2.Single factor analysis of secondary AP in patients with dysphagia after stroke:the observation group had tracheotomy,basic diseases with hypertension and/or diabetes,and the increase of age were risk factors for AP(P<0.05),but there was no correlation with sex(P>0.05).Logistic regression analysis was performed for statistically significant indicators:It was suggested that with age,tracheotomy and underlying diseases with hypertension and/or diabetes were risk factors for secondary AP in stroke patients with dysphagia.Conclusion:1.The incidence of dysphagia can be reduced by increasing the screening of the modified WATIAN drinking water test before the swallowing disorder training,and giving guidance on ingestion based on the results.2.In this study,the incidence of AP in stroke patients with dysphagia increased with age,the risk factors for AP are also high blood pressure and/or underlying diabetes and tracheotomy.
