| Objective: To study the diagnostic significance of 18F-fluorodeoxyglucose PET/CT(18F-FDG PET/CT)imaging in the diagnosis of lymph node metastasis,primary pathologic type and differentiation degree of ovarian malignant tumor Materials and methods: A total of 214 patients with gynecological ovarian malignant tumor in our hospital on January 1,2014 and December 30,2019 were selected,all of whom underwent the first stage surgery within 10 days after PET/CT.With the postoperative pathology as the standard reference,all of them were confirmed as ovarian malignant tumor by postoperative pathology.The relationship between pathological types,degree of differentiation and SUVmax of primary lesions was compared.The positive predictive value,negative predictive value,sensitivity,accuracy and specificity of PET/CT in the diagnosis of retroperitoneal lymph node metastasis were calculated.The ROC curve was drawn by SUVmax value at lymph nodes to determine whether SUVmax was related to lymph node metastasis.The optimal boundary value for PET/CT diagnosis of lymph node metastasis was determined by calculating the area under the curveResults: 1.No obvious difference exists in the SUVmax value of the primary tumor in preoperative PET-CT in the postoperative pathological type.2.The SUVmax value of primary lesions in preoperative PET-CT was positively correlated with the degree of postoperative pathological differentiation,and the difference was statistically significant(P<0.01).3.Among 104 patients whose SUVmax at the lymph node site was measured by PET/CT,63 cases were confirmed to be lymph node metastasis by pathology after operation,61 cases were successfully judged by PET-CT,and 41 cases were confirmed by pathology after operation without lymph node metastasis.PET-CT successfully diagnosed 28 cases.3.The area under the receiver operating characteristic(ROC)curve of preoperative lymph node SUVmax and postoperative pathologically confirmed lymph node metastasis was 0.905,P<0.05,the difference was statistically significant,and the best diagnostic cutoff value was 3.82.At this time,the sensitivity of lymph node metastasis was 95.2%,the specificity was 80.4%,and the accuracy was 89.4%.Conclusion: 1.The pre-operative SUVmax value of the primary focal PET/CT was correlated with the degree of postoperative pathological differentiation,and the higher the SUVmax value,the lower the degree of postoperative pathological differentiation.2.The preoperative PET/CT primary focal SUVmax had no predictive significance in different pathological types of ovarian malignant tumors.3.Preoperative PET/CT has a good diagnostic efficacy for retroperitoneal lymph node metastasis of ovarian cancer.4.In this study,the optimal cutoff value of SUVmax for PET/CT diagnosis of lymph node metastasis in our hospital was 3.82,and the diagnostic accuracy was higher when SUVmax≥3.82 was taken as the standard. |
