Objective: To investigate the relationship between plasma adiponectin level and the incidence,severity and prognosis of acute atherosclerotic cerebral infarction.Methods: A total of 136 patients with acute atherosclerotic cerebral infarction who were hospitalized in Department of Neurology,Shengjing Hospital,China Medical University from January 2020 to December 2020 were selected as the case group,and 105 healthy people who underwent physical examination during the same period were selected as the control group.Plasma adiponectin levels were determined by enzymatic linked immunosorbent assay(ELISA),and clinical data of all subjects were collected,and the relevant indexes were analyzed statistically.Result: Plasma adiponectin levels in patients with acute major atherosclerotic cerebral infarction were significantly lower than those in healthy controls(P < 0.001),and the difference was statistically significant.Logistic regression analysis was performed on the indicators with statistical significance between the case group and the control group.The results showed that age,smoking,hypertension,diabetes,homocysteine and LDL-C were independent risk factors for acute aortic atherosclerotic cerebral infarction(P < 0.05).Plasma adiponectin(OR=0.86;95% CI = 0.762,0.97;P=0.014)was a protective factor for aortic atherosclerotic cerebral infarction;Plasma adiponectin levels in healthy group,asymptomatic atherosclerosis group and case group had statistical significance(P < 0.05).In case group patients,if there is a vulnerable carotid plaques,the number of blood vessels of intracranial artery stenosis and infarction responsibility vascular stenosis degree has significant influence on plasma adiponectin levels,carotid vulnerable plaques group of plasma adiponectin level is lower than the group of vulnerable plaques,single artery stenosis group of plasma adiponectin level is higher than many artery stenosis group,The plasma adiponectin level in the group with moderate intracranial offending vessel stenosis was higher than that in the group with severe intracranial offending vessel stenosis,with statistical significance(P < 0.05).The plasma adiponectin level was negatively correlated with the rate of infarct offending vessel stenosis(r=-0.587,P <0.001).In patients with acute aortic cerebral infarction,the plasma adiponectin level in the mild neurological impairment group was higher than that in the moderate and severe neurological impairment group,and the difference was statistically significant(P <0.001).Correlation analysis showed that the plasma adiponectin level was negatively correlated with NIHSS scores at admission(r=-0.309,P < 0.001).The plasma adiponectin level in the group with infarct volume < 5ml was higher than that in the group with infarct volume ≥5ml,and the difference was statistically significant(P <0.001).Correlation analysis showed that plasma adiponectin level was negatively correlated with infarct volume(r=-0.307,P < 0.001).The plasma adiponectin levels in the first and second infarcts were compared.The results showed that the plasma adiponectin levels in the second infarcts group were significantly lower than those in the first infarcts group,and the difference was statistically significant(P = 0.001).Plasma adiponectin levels at admission were inversely correlated with 3-month MRS score in patients with acute aortic cerebral infarction(r=-0.301,P < 0.001).Conclusion: 1 、 Adiponectin is a protective factor for acute atherosclerotic cerebral infarction.Low levels of adiponectin increase the risk of acute atherosclerotic cerebral infarction.2、Plasma adiponectin level is closely related to atherosclerosis.3、 Plasma adiponectin level can be used as an indicator of severity and short-term prognosis in patients with acute atherosclerotic cerebral infarction.
