Objective:By analyzing the basic data features,causative agent species diversity and antibiotic tolerance diversity of patients with urolithiasis complicated with urinary tract infection,sum up the features,preventing actively and using drug rationally to improve the target populations’condition and improve the curative effect.Methods:A total of 480 patients diagnosed with urinary calculi and positive clean middle urinary culture during hospitalization in the section of Urology of Shaoxing Hospital of China Medical University from January 2015 to December 2019 were chosen as the experimental samples.Statistical verify the datum of cases information included:stone length and location,age,gender,basic diseases,etc.Analyze the characteristics of population distribution,causative agent species diversity and antibiotic tolerance diversity of urinary tract infection in various samples with urolithiasis according to the results of causative agent culture and antibiotic tolerance test.Results:(1)Among 480 urinary calculi combined with urinary tract infection(UTI),female 280 cases,accounting for 58.3%;male 200 cases,accounting for 41.7%;The mean age of the population was 55±15 years.There were 290 cases of Gram-negative bacilli(GNB),accounting for 60.4%;131 cases of Gram-positive Cocci(GPC),accounting for 27.3%;59 cases of Fungi,accounting for 12.3%.The top four dominating infected causative agent were E.coli,KPN,E.faecalis and E.faecium.(2)Among the 480 samples,the infection ratio of fungal was higher in person with diabetes mellitus,renal dysfunction,age>60 years old,white blood cells<4x10~9/L,C-reactive protein(CRP)≥10mg/L,and albumin<30g/L(P<0.05).Among the 480 samples,the infection rate of GNB was higher in women and patients with recurrent calculi(P<0.05).(3)The results showed that:Among 290 cases of GNB,the common GNB had high sensitivity to carbapenems andβ-lactamase inhibitors antibiotics,and had high tolerance to penicillins and cephalosporins.Among 131 cases of GPC,the common GPC had high susceptibility to linezolid,tegacycline and vancomycin;and had high tolerance to penicillin,erythromycin and clindamycin.Among 71 cases of fungi,Common fungi had high sensitivity to Amp B and 5-FU;and had certain degree of resistance to triazole fungicides.The tolerance ratio of fungi to Amp B and 5-FU were lower than those of triazole fungicides(P<0.01).Conclusions:Among person with urinary calculus complicated with UTI,female were more than male,elderly population was more than other age groups.Among the causative agent associated with urolithiasis,GNB were the major causative agent,among which E.coli was the the first prize.The patients with diabetes mellitus,renal dysfunction,age>60 years old,white blood cells<4x10~9/L,CRP≥10mg/L,and albumin<30g/L should be alert to fungal infection.The antimicrobial resistance of each pathogen is different.For the high-risk population of urinary calculus complicated with urinary tract infection,health education should be strengthened,active prevention should be taken,antimicrobial agents should be used rationally according to the consequence of drug susceptibility,and surgical means should be used to improve the patient’s condition and improve the efficacy when necessary.
