ObjectiveIn this study,a prospective randomized controlled clinical trial was conducted to compare the clinical efficacy of fire needle combined with cupping therapy and western medicine therapy in the treatment of acute herpes zoster.Record indicators of skin lesions,blood levels of il-10 and il-17 in patients with acute herpes zoster,to study the mechanism of fire needle combined with cupping therapy to promote the recovery of skin lesions.By comparing the incidence of postherpetic neuralgia with acupuncture combined with cupping therapy and western medicine therapy,the related factors influencing the occurrence of postherpetic neuralgia were studied.MethodsThis study will be in June 2018 to December 2019,the first affiliated hospital of guangzhou university of TCM acupuncture clinic,dermatology clinics,acupuncture wards of 60 in line with the diagnostic criteria,inclusion criteria,exclusion criteria of acute herpes zoster patients were randomized into treatment group and control group,30 patients in each group.The treatment group was treated with fire needle combined with cupping therapy once a day for a total of 10 days.The control group was given oral valaciclovir hydrochloride tablets(0.3g each time,twice a day)and vitamin B1 tablets(10mg each time,three times a day)for 10 days.The score of comprehensive efficacy of herpes zoster and skin lesions were recorded before and after each treatment.Serum levels of il-10 and il-17 were collected from peripheral blood before treatment and 11 days after treatment.The incidence of postherpetic neuralgia was recorded by follow-up at 30 days after treatment initiation.After the end of the study,the comprehensive curative effect,time to stop vesicles,time to scab,time to descab,incidence of postherpetic neuralgia after herpes zoster,serum levels of il-10 and il-17 were evaluated and analyzed.ResultsBaseline analysis: A total of 60 patients with acute herpes zoster were included in this study,30 in the treatment group and 30 in the control group.There were no statistically significant differences in age,gender,course of disease and general conditions between the two groups(P >0.05),indicating comparability.Comprehensive therapeutic effect: In the group of fire needle combined with cupping,3 cases were cured,18 cases showed obvious effect,8 cases were effective,and the total effective rate was 96.7%.In the western medicine group,1 case was cured,20 cases showed obvious effect,13 cases were effective,and the total effective rate was 80%.The difference of total effective rate between the two groups was statistically significant(P < 0.05).Skin lesions: The mean time to stop vesicles was 3.07±1.60 days,the mean time of scab formation was 5.13±2.23 days,and the mean time of descab was8.93±2.59 days.In the western medicine group,the mean time to stop vesicles was 4.27±1.99 days,the mean time to scab was 6.57±1.87 days,and the mean time to descab was 10.13±1.57 days.The difference between the two groups was statistically significant(P < 0.05).Inflammatory indicators: Before and after treatment,the mean serum levels of il-10 in the fire needle combined cupping group were 483.74±48.21pg/ml,413.59±34.26pg/ml,and the mean serum levels of il-17 were 39.78±7.72pg/ml,25.22±6.07pg/ml,respectively.Before and after treatment,the average serum il-10 level in the western medicine group was 501.45±29.20pg/ml,449.58±69.92pg/ml,and the average serum il-17 level was 43.09±8.30pg/ml,35.09±8.69pg/ml,respectively.There was a significant difference in serum il-10 and il-17 between the two groups before and after treatment(P < 0.05),which was statistically significant.There was no significant difference between the fire needle and cupping group and the western medicine group before treatment(P >.05),but the difference between the two groups after treatment was statistically significant(P < 0.05).Incidence of postherpetic neuralgia: The incidence of postherpetic neuralgia was 23.33% in the group of fire needle combined with cupping.The incidence of postherpetic neuralgia in western medicine group was 43.33%.The difference between the two groups was statistically significant(P < 0.05).Conclusionfire needle combined with cupping therapy in the treatment of acute herpes zoster has a definite effect.In promoting skin repair on better than western medicine treatment.fire needle combined with cupping therapy reduced the incidence of postherpetic neuralgia. |
