| Objective(1)Objective to establish a group of rat models simulating the natural course of steroid induced osteonecrosis of femoral head(SIONFH),so as to simulate the SIONFH rats in different stages of bone repair,the number,dispersion,thickness and volume fraction of trabecular bone in cancellous bone were obtained by Micro-CT scanning of the femoral head of rats.From the perspective of the microstructure changes of trabecular bone in the femoral head,the characteristics of the microstructure changes of trabecular bone in the whole process of bone repair in SIONFH rats were analyzed and discussed,so as to more accurately guide the clinical treatment of hip preservation;(2)Objective to observe and compare the effect of Fufang Shengmai Chenggu Capsule on the microstructure of bone trabeculae of femoral head in rats with SIONFH.MethodsSeventy male SD rats were randomly divided into control group(0w),2W,4W,6W,8W and 10 W.At the same time,another group of rats were injected with imiquimod and methylprednisolone every 2 weeks.The rats were given Fufang Shengmai Chenggu capsule by gavage.The femurs were taken after the experiment He staining and Micro-CT scanning were performed on the head mark;retain serum samples for ELISA testing;After the scanning,the three-dimensional reconstruction analysis of the scanned images was carried out by using NRecon software.The region above the greater trochanter of the femur was selected as the region of interest(ROI)to reconstruct the images by using dataviewer software.The microstructure of the cancellous bone of the femoral head in each group was quantitatively analyzed by using CTAn software.50 CT images 5mm above the growth plate line of the femoral head were selected for ROI quantitative analysis The main quantitative analysis indexes included trabecular volume fraction,trabecular thickness,trabecular number and trabecular dispersion.Results(1)The volume of trabecular bone(BV/TV)of cancellous bone of femoral head in rats with steroid induced osteonecrosis showed a trend of first decrease and then increase,and reached the lowest value(31.81 ± 5.278;%)at the fourth week of osteonecrosis,and began to rise at the sixth week(Tb.Th)At the 8th week,it reached the lowest value(0.05322 ±0.02067;/mm),And then began to rise;bone trabecular dispersion(Tb.Sp)The number of trabeculae(TB.N)decreased first and then increased,reaching the highest value(6.441 ±1.276;/mm)at the 8th week,and then decreased.(2)The ACP5 and CTSK proteins in the serum samples of SIONFH rats gradually increased in the first 6 weeks,reached the highest in the 6th week,and then decreased.(3)The trabecular volume of cancellous bone in the Fufang Shengmai Chenggu capsule group(41.62 ± 4.355;%)was significantly higher than that in the 6W group(32.21± 5.997;%),and the difference was statistically significant(P<0.01);the trabecular thickness of the femoral head in the Fufang Shengmai Chenggu capsule group(0.066 ±0.015;/mm)was also higher than that in the 6W group(0.054 ± 0.016;/mm),and the trabecular dispersion and trabecular number were also increased There was no statistical difference.Conclusions(1)In the first two weeks of steroid induced osteonecrosis of femoral head in SD rats,the volume and thickness of trabecular bone increased slightly,and the number of trabecular bone decreased slightly.The strength of the femoral head was higher at this stage.(2)From the second week to the eighth week of steroid induced osteonecrosis,the thickness of trabecular bone decreased gradually,but the number of trabecular bone increased,and the change of trabecular volume remained relatively stable.However,the volume of trabecular bone was in a weak state compared with the volume of trabecular bone in the 0W group.This stage was a high risk period of femoral head microfracture.(3)From the 8th week to the 10 th week,the volume and thickness of trabecular bone gradually increased,and the number of trabecular bone decreased.The data of the three groups were similar to that of the blank group.At this time,the strength of the femoral head had no significant difference with that of the blank group,and the bone repair was basically completed.(4)During the first 6 weeks of bone repair,the activity and bone resorption capacity of osteoclasts gradually increased,and then gradually decreased.At this time,it may be used to monitor or intervene in osteoclast nodes.(5)Fufang Shengmai Chenggu capsule can significantly increase the volume of trabecular bone in rats with steroid induced femoral head necrosis,and can effectively prevent and treat steroid induced femoral head necrosis. |
