ObjectiveThe staging of rectal cancer is related to the survival rate of patients,while the staging of rectal cancer is closely related to whether the lymph nodes of patients with rectal cancer have metastasizedd.Currently,more than 12 lymph nodes are required for rectal cancer patients,but sometimes there are insufficient numbers,This study analyzed the clinical data of 200 patients with rectal cancer who met the inclusion criteria,and explored the correlation between rectal cancer lymph node metastasis and the deficiency type,At the same time,analyze the risk factors that affect rectal cancer lymph node metastasis,in order to provide a basis for the clinical diagnosis and treatment of rectal cancer.MethodsAccording to the exclusion criteria,collected in March,2016-March 2020,the first affiliated hospital of guangzhou university of TCM anorectal rectal cancer resection in patients with clinical data,including patient gender,age,preoperative CEA,CA199,NLR,PLR,aggressive tumor(tumor size,tumor location,tumor differentiation degree,T staging of tumor,vascular tumor emboli),TCM syndrome type(deficiency and actuality),TCM symptoms(blood in the stool,incomplete stool,change in bowel habits,abdominal pain,bloating,weight loss),Use ROC curve to analyze the prediction of lymph node metastasis by NLR and PLR,select the cut-off value,and divide NLR and PLR into high and low grade two groups,using SPSS25.0 to perform single factor analysis and Logistic multivariate regression analysis on the above clinical data,to study the correlation between clinical features and lymph nodes of rectal cancer.ResultsA total of 200 patients were enrolled,of which 92 were lymph node metastases(46%);Univariate analysis showed that:The lymph node metastasis of rectal cancer was correlated with age(P=0.002),tumor size(P=0.045),tumor differentiation degree(P=0.001),tumor T stage(P < 0.001),tumor thromb in the vessel(P=0.003),preoperative CEA level(< 0.001),CA199 level(P=0.001),preoperative NLR(P=0.003),PLR(P=0.006),TCM syndrome type(P=0.005),abdominal pain and distension(P=0.001),and emaciation(P=0.005).There was no correlation with gender,tumor location,hematochezia,incomplete stool feeling and stool habit change(P > 0.05).Multivariate analysis showed that age,tumor differentiation degree,tumor T stage,NLR,CA199 level,TCM syndrome type(deficiency),abdominal pain and distension were independent risk factors for lymph node metastasis of rectal cancer(P < 0.05).ConclusionAge,tumor differentiation,depth of tumor invasion(T stage),NLR,CA199,TCM syndrome(deficiency),abdominal pain and abdominal distension are independent risk factors that affect rectal cancer lymph node metastasis.Studying the risk factors of rectal cancer lymph node metastasis is of great significance for formulating a reasonable surgical plan and postoperative treatment.
