Objective To explore the relevant factors influencing the phenomenon of weight retention in puerperium women, analyze the influence of dietary intake on the changes of puerperium weight and body mass index,in order to provide a scientific and effective reference for further reducing the weight retention of women after delivery. Methods The study selected women who had undergone puerperal rehabilitation at Beijing United Family Rehabilitation Hospital. The retrospective cohort analysis method was used to investigate the general information of the subjects, breastfeeding methods during the puerperium, exercise frequency, total length of sleep,as well as pre-pregnancy weight,pre-natal weight,Physical examination information such as body weight at different time points within 42 days of postpartum and calculation of body mass index(BMI);a 24-hour diet review method was used to regularly collect the dietary intake of the study subjects.Through statistical analysis of single-factor variance,at the same time,includingχ~2 test and multiple linear regression and other related statistical methods, the influencing factors of women’s weight retention for 42 days postpartum were explored and analyzed.Results(1)There were 591 participants in our study. At 7 days,14 days,21 days,28 days, and 42 days after delivery, the average body weight retention of the subject was8.88±4.31kg,6.43±4.36kg,5.34±4.73kg,4.76±4.78kg and 4.01±4.33kg;the average BMI retention was 3.29±1.82kg/m~2,2.38±1.82kg/m~2,1.98±1.81kg/m~2,1.76±1.83kg/m~2,1.48±1.62kg/m~2;The rate of returning to pre-pregnancy weight is3.72%and 10.83%,17.09%,31.64%and 37.73%;the rates of overweight and obesity were 29.6%,28.9%,27.2%,22.1%,18.2%.(2)Through single-factor and multi-factor logistic correlation analysis, the maternal BMI value before pregnancy(OR=1.383,95%CI:1.091~1.753,P<0.05),weight gain during pregnancy(OR=1.740,95%CI:1.122~2.699),P<0.05),exercise frequency(OR=0.630,95%CI:0.466~0.855,P<0.05),feeding method(OR=1.692,95%CI:1.281~2.235,P<0.001),sleep duration(OR=0.531,95%CI:0.523~0.710,P<0.05),the super-recommended value of the energy supply ratio of fat(OR=1.527,95%CI:1.129~2.964,P<0.05),the energy supply ratio of carbohydrate The over-recommended values(OR=1.409,95%CI:1.107~1.793,P<0.05)are all related factors influencing women’s weight retention during the puerperium.(3)Compared with control group women, women who received dietary intervention were evaluated for their weight retention status, body mass index(BMI),waist circumference, hip circumference,waist-to-hip ratio at each designated follow-up time node relative to their baseline weight. Results The decrease values are significantly increased. Conclusion In this study, the weight retention and BMI retention of women during the puerperium showed an overall downward trend, but overweight and obesity were more common, and two-thirds of the women at 42 days could not recover their pre-pregnancy weight. While the women’s postpartum weight retention and their pre-pregnancy BMI value, weight gain during pregnancy, exercise frequency, postpartum feeding method, fat and carbohydrate energy supply ratio exceeding the recommended value and many other factors are closely related, control the whole pregnancy weight within the appropriate range, strengthen breastfeeding, ensure adequate sleep and appropriate postpartum exercise All can reduce postpartum weight retention;according to the perinatal situation of women,if timely and effective dietary intervention measures are taken for puerperium women,a reasonable diet plan and weight self-management plan can be formulated to reduce postpartum weight retention and help postpartum mothers.Eat a scientific diet and restore a suitable weight faster.
