| Objectives To investigate the influence of ideal cardiovascular health behaviors and factors on malignant tumor of digestive system in northern China.Methods The design method of prospective cohort study was adopted.A total of 91857staffs who participated in the health checkup of the Kailuan Hospital for the first time from July 2006 to June 2007,and the incidence of malignant tumor of digestive system was followed up and collected.According to the cardiovascular health score,the subjects were divided into three groups:25742 cases in the first quantile group(0-7),49965 cases in the second quantile group(7-10),and 16150 cases in the third quantile group(10-14).The life-table method was used to calculate the cumulative incidence of malignant tumor of digestive system in each group,the incidence rate of the observed population was calculated,the difference of cumulative incidence among each group was compared by Log-Rank test.The Cox regression model was used to analyze the influence of cardiovascular health score on malignant tumor of digestive system.And analyze the effect of any component of ideal cardiovascular health behavior and factors on the incidence of digestive system malignant tumor.Results During the average follow-up of 11.49 years,there were 1496 cases of digestive system malignant tumors,the incidence density was 1.42/1000 person-years,and the cumulative incidence rate was 1.71%.The cumulative incidence rates of the three groups were 1.90%,1.79%and 1.18%,respectively.Log-Rank test showed that there was significant difference among the three groups(χ~2=35.63,P<0.001).The incidence rates among the three groups were 1.59/1000 person-year,1.48/1000 person-year and 0.96/1000person-year,respectively.The results of Cox proportional hazard model showed that after correcting the confounding factors,with the first quantile as the control group,the HR of the second and third quantile were 0.96(95%CI:0.85-1.08)and 0.80(95%CI:0.66-0.95),respectively.In the gender stratification between men and women,there was a significant statistical difference only in the male population.Compared with the corresponding non-ideal factors,the HR of ideal diet,ideal blood glucose,ideal smoking digestive system malignant tumor were 0.90(95%CI:0.83-0.98),0.91(95%CI:0.84-0.98)and 0.93(95%CI:0.87-0.99),respectively.Conclusions The higher scores of ideal cardiovascular health behaviors and factors play a protective role in the pathogenesis of malignant tumor of digestive system,and the risk of malignant tumor of digestive system decreases with the increase of cardiovascular score.Ideal diet,ideal fasting blood sugar and ideal smoking play a protective role in the incidence of digestive system malignant tumors.Figure 2;Table 10;Reference 114... |
