Objectives To explore whether bedside ultrasound monitoring of gastric antral motility can be used as an effective examination of the degree and prognosis of gastrointestinal dysfunction in severe patients.Methods All the patients who met the inclusion criteria and were admitted to the ICU for6 consecutive months from July 2020 to December 2020 were fasted overnight for 8 hours and the gastric antral area was measured by bedside ultrasound and the antral contraction frequency(ACF),antral contraction amplitude(ACA)and the antral motility index(MI)were calculated by the formula.At the same time,the patient’s corresponding clinical data,such as acute physiology and chronic health evaluation(APACHE II score)II,sequential organ failure estimation score(SOFA score),nutrition risk screening score(NUTRIC score),intra-abdominal pressure(IAP)and laboratory results,examination results,clinical manifestations,vital signs,hospital admission diagnosis,ICU diagnosis,whether after surgery,etc,which were recorded.Subsequently,the SPSS26.0 version was used for statistical analysis of the above data.Results In the 6-month study,according to the inclusion criteria,a total of 60 patients underwent routine gastric antral motion ultrasound examination.According to the acute gastrointestinal injury(AGI)classification,30 patients were classified into AGI Ⅰgrade,18 into AGI Ⅱgrade,9 into AGI Ⅲgrade,and 3 into AGI IVgrade.After,using Kruskal Wallis test,Spearman rank correlation analysis and ordered Logistic regression analysis,(1)the non-parametric test results of k independent samples showed that the P values of ACA,ACF,and MI were<0.05.The monitoring of gastric antral motility by bedside ultrasound was consistent with that of gastrointestinal dysfunction in severe patients,and the results are statistically significant;(2)Speaman correlation analysis showed that ACA,ACF,MI were significantly correlated with AGI grading(r=-0.441,-0.471,-0.481,P<0.001);(3)MI was negatively correlated with APACHE Ⅱ score(r=-0.565),SOFA score(r=-0.314),Nutric score(r=-0.340)and IAP(r=-0.396)(P<0.05).(4)MI was negatively correlated with the length of stay in ICU(r=-0.363)(P<0.05),and there was no correlation between MI and length of stay in the hospital(P>0.05).Conclusions Bedside ultrasound monitoring of gastric antral motion(ACA,ACF,MI)is correlated with AGI grading,which can be used to assessment the severity of gastrointestinal functional dysfunction.Figure[3];Table[6];Reference[170]...
