Objectives Explore the expression of plasma SHOX2 and RASSF1A gene methylation in malignant pulmonary nodules,and to analyze the relationship between the expression of plasma SHOX2 and RASSF1A gene methylation and the clinical characteristics of patients with malignant pulmonary nodules,including tumor markers,pulmonary nodule imaging and partial immunohistochemistry.Methods From December 2019 to December 2020,68 patients with pulmonary nodules diagnosed by chest CT in Department of respiratory medicine and Department of cardiothoracic surgery in Affiliated Hospital of North China University of technology were selected;The pathological types of pulmonary nodules were determined by percutaneous lung puncture,bronchoscopy,thoracoscopy and surgery.According to the pathology,47patients with malignant pulmonary nodules were divided into case group(malignant);21patients with benign pulmonary nodules were selected as control group(benign);SHOX2and RASSF1A gene methylation in peripheral venous blood of 68 patients with pulmonary nodules were detected by sulfite modification and Taq Man probe.Some of the results of immunohistochemistry were obtained,and the basic clinical information of the patients was collected,including chest high-resolution CT,tumor markers,D-dimer and other auxiliary examinations.The relevant information was input into excel and analyzed by spss25.0 data software.χ~2 test was used to compare the qualitative data among groups,and the percentage(%)was calculated.Logistic regression analysis was used for multivariate analysis.The test level wasα=0.05,P<0.05 was statistically significant.Results 1 The CEA of tumor markers,imaging features including burr sign,vascular bundle sign,pleural indentation sign,lobulation sign,nodule density as ground glass nodule,and plasma SHOX2 and RASSF1A gene methylation were significantly different between the two groups(P<0.05).There were no significant differences in age,BMI,gender,smoking history,family history of tumor,AFP,NSE,CA199,CA724 in tumor markers,D-dimer,and the largest diameter of pulmonary nodule,satellite focus,cavity sign,nodule location between the two groups(P>0.05).2 Multivariate logistic regression analysis showed that the CEA of tumor markers(OR=0.043,P=0.022),ground glass nodules(OR=0.068,P=0.039)and methylation of RASSF1A gene in plasma(OR=0.102,P=0.040)were the independent factors to judge malignant pulmonary nodules.3 There was no significant difference between the expression of SHOX2 and TTF-1、Napsin A、CK7、p53(P>0.05);there was a significant negative correlation between the expression of RASSF1A and TTF-1(rs=-0.324,P<0.05),Napsin A(rs=-0.306,P<0.05),and there was no significant difference between the expression of RASSF1A and CK7、p53(P>0.05).Conclusions 1 The methylation of RASSF1A gene in plasma,the CEA of tumor markers and imaging features of ground glass nodules were independent factors of early malignant pulmonary nodules.2 Patients with early malignant pulmonary nodules with positive methylation of RASSF1A gene in plasma were often accompanied by negative expression of TTF-1 and Napsin A in immunohistochemistry.Figure0;Table8;Reference 130...
