| Objectives To explore the clinical value of multimodal ultrasound combined diagnosis in the diagnosis of prostate lesions,through the analysis of the sensitivity,specificity and accuracy of multimodal ultrasound in the diagnosis of prostate lesions,to provide reliable imaging reference for clinical departments to make reasonable diagnosis.Methods From October 2019 to February 2021,78 patients with 87 lesions underwent prostatectomy in North China University of Science and Technology.Conventional Transrectal ultrasound(TRUS),contrast enhanced ultrasound(CEUS)and shear wave elastography(SWE)were performed before operation.We observed the size,echo,boundary,shape,strong echo and other indicators of the nodules under conventional ultrasound,and analyzed them;we measured the young’s modulus of prostate lesions(Emax,Emean,Esd)through Q-box software,and analyzed their elastic values;we observed the enhancement characteristics of prostate lesions under contrast-enhanced ultrasound,and drew the time-intensity curve(TIC)through the system.The enhancement intensity(EI),time to peak(Tp)and area under time intensity curve(AUCt)were measured.The counting data were expressed by frequency or rate and analyzed by chi square test.Kolmogorov Smirnov test was used to test whether the measurement data obeyed the normal distribution.If the measurement data obeyed the normal distribution,it was expressed by sx ±,The independent sample t test was used for statistical analysis.If the measurement data did not obey the normal distribution,it was expressed by interquartile [M(QR)] and analyzed by Mann-Whitney U test.The receiver operating characteristic(ROC)curve was drawn with pathological results as the gold standard,and the cut-off point of benign and malignant prostate lesions was analyzed,and the sensitivity,specificity and accuracy were calculated.In addition,binary logistic regression analysis was used to establish the regression equation for us,SWE and CEUS.Results 1 General data: there were 78 patients with an average age of 68.7±6.7 years(range,51~71 years),including 87 lesions.According to the nature of prostate lesions,they were divided into benign group and malignant group,including 42 benign nodules and 45 malignant nodules.2 Results of two-dimensional ultrasound: the signs of prostate malignant lesions included hypoechoic,lobulated or irregular,fuzzy boundary,invasion to the outside of the prostate and punctate hyperechoic in the lesions,the differences were statistically significant.3 Results of shear wave elastography: the elasticity values of Emax、Emean and Esd in malignant lesions were higher than those in benign lesions,and the cutoff values were 93.6kpa,89.5kpa,5.5kpa respectively.4 Results of contrast-enhanced ultrasound: the main enhancement features of prostate malignant nodules were uneven high enhancement,unclear boundary after enhancement,enlarged lesions after contrast,synchronous enhancement with normal prostate tissue,and synchronous regression;the EI and auct of prostate malignant lesions were higher than those of benign nodules,and Tp was lower than those of benign lesions.5 Multimodal ultrasound: the sensitivity,specificity and accuracy of multimodal ultrasound were 88.9%,85.7%,87.4%.Conclusions 1 Multimodal ultrasound has high sensitivity,specificity and accuracy in the prediction of prostate lesions.It can give full play to the diagnostic advantages of us,SWE and CEUS,and improve the diagnostic efficiency of prostate lesions.2 TRUS,SWE and CEUS have certain clinical value in the diagnosis of benign and malignant prostatic lesions,but the sensitivity,specificity,sensitivity of a single technique are lower than those of multimodal ultrasound.Figure6;Table18;Reference150... |
