| Objectives To investigate the clinical efficacy and safety analysis of ultrasound-guided lauromacrogol combined with 50% glucose sclerotherapy in the treatment of ovarian endometriosis cyst.Methods From October 2018 to February 2020,42 patients with OEC treated by ultrasound-guided lauromacrogol combined with 50% glucose sclerotherapy in Tang Shan Gong Ren hospital were selected as the observation group;another 45 patients with OEC treated by laparoscopic exfoliation in this hospital at the same time were selected as the control group.Questionnaire was made to collect the basic data of the research objects.Preoperative routine ultrasound examination was performed to record the location,maximum diameter,internal echo,cyst wall thickness and other indicators of the cyst,and calculate the cyst volume based on the maximum diameter.The levels of FSH,LH and E2 were recorded before and after treatment.The serum CA125 levels were recorded before treatment and 1,3,6 months after treatment.The treatment time,operation cost and intraoperative blood loss were recorded.The adverse reactions was recorded.The diameter of cysts in the observation group was measured at 3,6 and 12 months after treatment.Application of SPSS 22.0 software statistical analysis.The measurement data were expressed with(sx ±),and the counting data is expressed with frequency or rate.Rrepeated measurement ANOVA was selected for comparison of repeated measurement data,independent samples t test and Rank Sum was selected for comparison measurement data.Paired t test was used when the preoperative and postoperative changes in the group were compared with normal distribution.The comparison between classified count data was used Chi-square test,and Rank Sum test was used for the comparison of rank data.Results 1 General information: The average age of the observation group was 36.76 ±7.09 years(range,25-49 years),while that of the control group was 34.58 ± 6.81 years(range,22-47 years).There were no statistical differences in age,mean diameter and volume of cysts,marital status,initial recurrence,location of lesions and clinical symptoms between the 2 groups(P>0.05).2 Operation: The observation group had better surgical time,intraoperative bleeding,and surgical cost than the control group,and there were statistical differences(P<0.05).3 Curative effect analysis: The volume reduction rates of the observation group reached(77.11±24.66)%,(86.88±20.57)%,and(90.16±16.90)% at 3months,6 months,and 1 year after treatment,and the cure rates reached 61.90%,71.43%,and 76.19%,respectively.The total effective rate reached 85.71%,88.10%,and 90.48% in that order.In the control group,the cure rate was 82.22%,80.00%,77.78%,and the total effective rate was 95.56%,93.33%,86.67% at the three time points,respectively.There was no significant difference in the efficacy and total effective rate at other time points between the two groups except that the efficacy of the control group was better than that of the observation group at 3 months after treatment.The recurrence rate was 13.33% in the observation group and 22.22% in the control group.4 Serum CA125 level: The serum CA125 level of the two groups decreased to normal with time,but the degree and trend of the two groups were not consistent with time.With the control group decreasing more rapidly to normal one month after treatment,and the observation group decreased to normal three months after treatment.5 Ovarian function: The differences in LH,FSH,and E2 between the two groups before treatment were not statistically significant(P>0.05).But after treatment,LH and FSH were lower and E2 was higher in the observation group than in the control group,the difference between the two groups after treatment was statistically significant(P<0.05).The differences in LH,FSH,and E2 in the observation group after treatment were not statistically significant compared with those before treatment(P>0.05),the levels of FSH,LH and E2 in the control group were significantly different from those before treatment(P<0.05).Conclusions 1 Compared with laparoscopic cystectomy,ultrasound-guided lauromacrogol combined with 50% glucose sclerotherapy has the advantages of less trauma,shorter operation time,lower cost,no hospitalization and less adverse reactions.2 The serum CA125 level was significantly decreased after operation.3 Ultrasound guided lauromacrogol combined with 50% glucose sclerotherapy has less effect on ovarian function than laparoscopic cystectomy.4 Ultrasound guided lauromacrogol combined with50% glucose sclerotherapy is effective in the treatment of OEC,with low recurrence rate,economy and safety,and has wide clinical application value.Figure10;Table10;Reference127... |
