Objectives Diffusion tensor imaging(DTI)is used to evaluate the degree of corticospinal tract injury caused by hematoma after moderate or small amount of intracerebral hemorrhage to guide the treatment of patients.Methods Small and medium volume(15ml~30ml)patients with cerebral hemorrhage in basal ganglia region admitted to the Department of Neurosurgery in the Affiliated Hospital of North China University of Science and Technology from September 2019 to December 2020 are collected as the research subjects.All patients are examined by diffusion tensor imaging within 72 hours after admission.According to the degree of corticospinal tract injury,the patients with the degree of injury 1~2 are divided into the experimental group(minimally invasive surgery group),32 cases are classified as A1 group,and the control group(conservative treatment group),35 cases are classified as B1 group.The muscle strength recovery of the affected side after 2 weeks of treatment is compared,and the living ability of the patients after 3 months is evaluated by m RS score.The patients with grade 3 injury are divided into the experimental group,24 cases were classified as group A2,and the control group,22 cases were classified as group B2.The muscle strength recovery of the affected side after 2 weeks of treatment is compared,and the living ability of the patients after 3 months is evaluated by m RS score.Results Through analysis and comparison of experimental group A1 and control group B1,it is found that there are no significant differences in hospitalization time,hematoma volume,muscle strength of affected side and blood pressure at admission(P >0.05).After2 weeks after treatment compared with lateral muscle A1 group is significantly higher than B1,the difference is statistically significant(P < 0.05),3 months pay a return visit patients using m RS score evaluation of life of patients with self-care ability,three months after the two groups were compared life self-care ability,found that patients with minimally invasive surgery group life ability better than conservative treatment group of patients.Comparison of experimental group A2 and control group B2 showed that there are no significant differences in hospitalization time,hematoma volume,muscle strength and blood pressure(P >0.05).After 2 weeks after treatment compared with lateral muscle A2 and B2 group has obvious no difference,after 3 months pay a return visit patients using m Rs score evaluation ability of the patient’s life,three months after the two groups are compared life ability,found that patients with minimally invasive surgery group life ability compared with conservative treatment group have obvious no difference.Conclusions For patients with corticospinal tract displaced by compression and a small part of interruption,the efficacy of minimally invasive surgical treatment is better than that of patients with conservative treatment.Conservative treatment is feasible for most or all of the patients with severe corticospinal tract injury,because minimally invasive surgical treatment does not improve the prognosis of the patients.Figure 18;Table 4;Reference 153...
