| ObjectiveThrough the questionnaire survey in elderly patients with surgical operation,to study the status of the preoperative asthenia syndrome and to explore risk factors,to establish a more suitable for elderly patients with surgical field multidimensional preoperative asthenia syndrome provides the basis for evaluation method and nursing measures and the suggestion,improve the old patients with surgical perioperative nursing,reduce the occurrence of postoperative complications,improve the elderly patients quality of life.MethodThis study is a cross-sectional study.Select two 5 hospitals in the province of hubei and henan surgical department in elderly patients with preoperative as the research object,using the general data questionnaire,weak(FRAIL)scale,instrumental daily life activities ability scale,everyday life self-care ability and scale,mini nutritional assessment scale version Jane and simple mental state examination scale on a questionnaire survey was conducted in elderly patients with surgical,Excel software was used to input the data,and SPSS20.0 statistical software was used to conduct descriptive analysis and chi-square test on the data.All variables with statistical significance obtained after univariate analysis were included in disordered multi-classification Logistic regression,and the main risk factors of preoperative fission syndrome in elderly surgical patients were screened out.Results1.There were 367 questionnaires,of which 342 were effective,with effective recovery rate of 93.2%.2.The incidence of frailty syndrome in elderly surgical patients before surgery was24.9%,and the incidence of frailty syndrome in early stage was 49.1%.The descending order of each index of the frailty scale was fatigue,increased resistance/decreased endurance,weight loss,decreased free movement,and coexistence of multiple diseases.3.The preoperative scores of instrumental daily living ability of elderly surgical patients were(20.96±5.07),and the patients with reduced instrumental daily living ability accounted for 59.8%.4.The scores of self-care ability of the elderly surgical patients before operation were(93.9±9.92),and the cases of self-care ability of daily living decreased by 32.1%.5.The average score of preoperative nutritional status in elderly surgical patients was(11.03±2.94).The patients with good nutritional status accounted for 61.7%,the patients with malnutrition risk accounted for 23.7%,and the patients with malnutrition accounted for 14.6%.6.The average score of preoperative simple mental state examination scale in geriatric surgical patients was 23.97±3.38 points.The patients with cognitive impairment accounted for 38.3%,and the patients with normal cognition accounted for72.7%.7.According to the results of univariate analysis,gender,age,pre-retirement occupation,economic status,BMI(Body Mass Index),combined chronic diseases,instrumental daily living ability,self-care ability of daily living,nutritional status and cognitive function were compared with different flimsy states with statistical significance(P < 0.05).8.Multivariate disordered Logistic regression analysis showed that age,BMI,instrumental daily living ability,nutritional status,and cognitive status were the main influencing factors of preoperative fission syndrome in geriatric surgical patients.Aging,increased BMI and malnutrition were the risk factors for preoperative frailty syndrome in elderly surgical patients.Normal instrumental daily living ability,good nutritional status and good cognitive function were protective factors for preoperative frailty syndrome in elderly surgical patients.Conclusions1.Preoperative prevalence of frailty syndrome in geriatric surgical patients is not optimistic and should be paid more attention.2.Elderly surgical patients with aging,obesity and two or three chronic diseases are more likely to suffer from frailty syndrome.Factors such as normal instrumental daily living ability,good nutrition and good cognitive function are protective factors of frailty syndrome in elderly surgical patients.3.Clinical nursing staff should pay attention to preoperative elderly surgical patients with debilitating condition,as soon as possible for the elderly surgical patients for preoperative evaluation of asthenia syndrome,identify potential risk factors,and from multidisciplinary angles preoperative nursing measures timely optimization,prevent or reduce asthenia syndrome,in order to reduce the occurrence of postoperative adverse events. |
