Research purposeThrough statistical analysis of various data of patients with adenomyosis and healthy physical examination,menstruation,pregnancy and childbirth history,intrauterine device application,history of gynecological diseases and emotional factors,explore the correlation of adenomyosis influencing factors and provide a scientific basis for the prevention and treatment of the disease.Research methodsA retrospective analysis of 197 patients in the Second Hospital of Shandong University and the Maternity and Child Health Hospital of Tengzhou City from January 2015 to December 2020(finally supported by pathology for AM diagnosis)was used as the AM group,and the same period was matched(2015 January From January to December 2020)206 healthy women undergoing physical examination at the Second Hospital of Shandong University and Tengzhou Maternal and Child Health Hospital served as the control group.Collect applicable case data in the form of a questionnaire survey(see Appendix 1),including general demographic information(related to age,education level,smoking history,etc.),menstruation,pregnancy and childbirth history,IUD application,gynecological diseases,Body mass index,personal emotional factors,etc.Analyze the content involved in the above-mentioned influencing factors of the AM group and the control group,preprocess the relevant data,and finally use SPSS 26.0 version software to analyze the data and draw the research conclusions.For the ordered classification data in the research data,the rank sum test in the statistical method is used to analyze the difference of the component data.For unordered classification data in the data,perform a chi-square test or use Fisher’s exact probability test for analysis.Binary Logistic regression analysis was used to statistically analyze the influencing factors of AM.When P<0.05,the difference was considered to be statistically significant.Research result(1)The results of univariate analysis of the influencing factors of AM:the distribution of the AM group and the control group at the cultural level(low level;medium level;high level),the distribution of dysmenorrhea(no dysmenorrhea,dysmenorrhea),Distribution of induced abortion history(no history of induced abortion,history of induced abortion),number of abortions(0 times,1 time and ≥2 times),distribution of IUD application(not used,LNG-IUS,IUD without progesterone),IUD time without progesterone(<5 years,5-10 years,>10 years),the distribution of endometriosis history(yes,no),the history of uterine fibroids(yes,no),The distribution of emotional factors(no anxiety,mild anxiety,moderate-severe anxiety)was significantly different(P<0.05).(2)Multi-factor analysis results of the influencing factors of AM:Compared with higher education level(OR=0.405,95%CI 0.197-0.832,P=0.014)and middle education level(OR=0.338,95%CI 0.199-0.574,P<0.001),patients with lower education level are more likely to Occurrence of AM lesions;dysmenorrhea(OR=3.203,95%CI 1.921-5.339,P<0.001)is more likely to affect AM lesions than no dysmenorrhea;history of artificial abortion(OR=1.852,95%CI 1.131-3.031,P=0.014)The level of AM lesions is higher than that of no history of miscarriage.In terms of IUD application,patients using IUD without progesterone(OR=2.558,95%CI 1.458-4.488,P=0.001)are more likely to cause AM.In addition,LNG-IUS(OR=0.346,95%CI 0.159-0.750,P=0.007)application will not lead to the development of AM,a history of endometriosis(OR=1.960,95%CI 1.036-3.707,P=0.038)and uterine fibroids Patients with(OR=2.693,95%CI 1.635-4.435,P<0.001)are more likely to develop AM than those without the medical history.In terms of anxiety,mild anxiety(OR=3.523,95%CI 2.105-5.896,P<0.001))And moderate-severe anxiety(OR=3.681,95%CI 1.294-10.470,P=0.015)are more likely to develop AM lesions than those without anxiety.Research conclusions1.Low education level,dysmenorrhea,history of induced abortion,history of endometriosis and history of uterine fibroids,IUD application type(without progesterone),anxiety are the influencing factors of AM.2.The longer the progesterone-free IUD is placed,the more the number of induced abortions can be regarded as the potential influencing factors of AM.3.Middle and high education level and LNG-IUS application are the protective factors of AM.4.By comprehensively analyzing the data of AM patients and healthy controls,this study better discusses the relevant influencing factors of am.It is found that the application of IUD without progesterone,low educational level and anxiety are new influencing factors of am,which provides new ideas for the prevention and treatment of AM patients and has important clinical guiding significance.
