Study On The Clinical Characteristics And Distribution Of Traditional Chinese Medicine Syndrome Types In 100 Patients With Endometriosis

Objective To collect the information of patients with endometriosis,summarize their clinical characteristics,explore the risk factors of the disease and the distribution of TCM syndrome types,and provide a basis for clinical prevention and treatment of endometriosis.Methods One hundred patients with postoperative pathological diagnosis of endometriosis in our inpatient and outpatient department were selected as the case group,and one hundred non-endometriosis patients of the same age range were selected as the control group.The questionnaires were filled out uniformly and processed with statistical software to study the clinical characteristics of endometriosis and the distribution of TCM syndrome types.Results 1.The rate and degree of dysmenorrhea before opreation in the case group was higher than that in the control group（P<0.05）,while there was no statistical difference in preoperative dysmenorrhea degree between patients with different clinical stages in the case group（P>0.05）,and no statistical difference in preoperative dysmenorrhea degree between patients with and without recurrence（P>0.05）.2.Compared with the control group,menstruation in case group was characterized by dysmenorrhea,hypermenorrhea,prolonged menstruation,thick sticky menstrual flow and often accompanied by blood clots（P<0.05）.3.The preoperative average CA125 level in the case group was higher than that in the control group（P<0.05）.Patients with clinical stage Ⅲ and Ⅳ had higher average CA125 levels than those with stage Ⅰ and Ⅱ,and the positive rate of CA125 increased with the increase of clinical stage（P<0.05）,After surgery,the average CA125 level decreased to negative range（P<0.05）,but there was no statistical difference in the preoperative positive rate of CA125 between patients with and without recurrence（P>0.05）.4.The proportion of patients with infertility in case group was higher than control group（P<0.05）,and 71.1%of patients in case group had infertility for more than 2 years.5.The clinical stages of patients who with abnormal menstruation,dysmenorrhea，discovery of mass and infertility as cardinal symptom were mostly stage Ⅲ and stageⅣ,while the clinical stages of patients who were diagnosed unexpectedly during other surgery and without obvious clinical symptoms were mainly Stage I（P<0.05）.6.Logistic showed that reproductive age,low BMI,,alcohol consumption,bad menstrual habits,rashness,anxiety,excessive consumption of greasy and raw or cold foods and multiple abortion may be risk factors for endometriosis,while multiple births may be protective factors.7.The main manifestations of blood stasis are pain,mass and bleeding,which are consistent with the clinical characteristics of endometriosis.8.The TCM syndromes types of 100 endometriosis patients were in the following order:kidney deficiency and blood stasis syndrome（43.0%）,qi stagnation and blood stasis syndrome（19.0%）,syndrome of stagnant dampness-heat（13.0%）,qi deficiency and blood stasis syndrome（10.0%）,syndrome of cold congeal and blood stasis blood stasis（9.0%）,syndrome of intermingled phlegm（6.0%）.And the patients with kidney deficiency and blood stasis syndrome had the highest proportion of infertility（46.7%）and dysmenorrhea（93.0%）.Conclusion 1.Abnormal menstruation,mass and infertility are the clinical characteristics of endometriosis.2.Serum CA125 level plays a certain role in the auxiliary diagnosis,clinical staging and efficacy evaluation of endometriosis.3.Reproductive age,low BMI,high education level,alcohol consumption,bad menstrual habits,mental disorder,dietary imbalance and abortion may be risk factors for endometriosis,while multiple births may be protective factors.4.Blood stasis is the basic pathogenesis of endometriosis.And the TCM syndrome types of the 100 endometriosis patients were in the following order from highness to lowness:kidney deficiency and blood stasis syndrome,qi stagnation and blood stasis syndrome,syndrome of stagnant dampness-heat,qi deficiency and blood stasis syndrome,syndrome of cold congeal and blood stasis blood stasis,syndrome of intermingled phlegm.
