Study On The Distribution And Changes Of TCM Syndromes In Advanced Colon Cancer Before And After Chemotherapy

Objective:By collecting the general data of patients with advanced colon cancer before and after chemotherapy,and the information of the contents,symptoms and signs of TCM diagnosis of advanced colon cancer,this paper discusses the distribution and changes of TCM syndromes and syndrome types of advanced colon cancer before and after chemotherapy.Methods:Case sources: 100 patients with advanced colon cancer who received chemotherapy in the Oncology Department of Hubei Integrated traditional Chinese and Western Medicine Hospital from January 2020 to February 2022,58 males and 42 females.The clinical data and TCM syndrome distribution information of the patients were prospectively collected,and the data were analyzed by SPSS25.0software.The TCM syndromes and syndrome types before and after chemotherapy were compared by chi-square test.The related factors of TCM syndrome type were analyzed by Pearson or Spearman correlation analysis method,and the common syndrome,syndrome type distribution and change characteristics before and after chemotherapy for advanced colon cancer were summarized,and some syndrome type related factors were discussed,so as to sum up the distribution change law of syndrome and syndrome type and the relevant factors with statistical significance and clinical significance.Results:1.The common symptoms before and after chemotherapy were fatigue,abdominal distension or pain,anorexia,nausea and vomiting,limb numbness,pale complexion,dry stool,thin stool,irregular stool,bloody stool or black stool,poor sleep,abdominal mass,anal distension,waist soreness,dizziness,bitter mouth,sallow complexion,dry mouth,abdominal pain,abdominal distension,night sweat,palpitation,warm and warm pressing,purple tongue,pale tongue.Red tongue,yellow tongue,greasy tongue,thick tongue,white tongue,fat tongue,tooth mark tongue,old tongue,tender tongue,sublingual collaterals（cyan,lilac）,slippery pulse,sluggish pulse,deficiency fine pulse,number pulse,astringent pulse,string pulse.2.The statistically significant syndrome changes before and after chemotherapy were as follows:（1）Fatigue,anorexia,nausea and vomiting,limb numbness,spontaneous perspiration,poor sleep pale complexion,thin stool,dizziness,night sweats,long urination and Excessive urine output,palpitation,the abdomen which Liked to be warmed and knead,light mouth,frequent nocturnal urination,shortness of breath,white moss,greasy tongue coating,fat tongue,toothed tongue,tender tongue,lilac sublingual veins,Sunken slow veins,Weak and small veins,increased significantly compared with those before chemotherapy（P < 0.05）.（2）The dry knot of stool,yellow fur,thick fur,blue and purple sublingual veins and slippery veins decreased significantly as compared with those before chemotherapy（P < 0.05）.3.Before chemotherapy,damp-heat syndrome was the most common syndrome,followed by blood stasis syndrome,spleen and kidney yang deficiency syndrome,spleen deficiency and liver depression syndrome,qi and blood deficiency syndrome,liver and kidney yin deficiency syndrome.After chemotherapy,spleen and kidney yang deficiency syndrome was the most common,followed by qi and blood deficiency syndrome,blood stasis syndrome,spleen deficiency and liver depression syndrome,damp-heat syndrome,liver and kidney yin deficiency syndrome.The changes of syndrome types before and after chemotherapy were statistically significant:（1）The syndrome of yang deficiency of spleen and kidney and deficiency of qi and blood after chemotherapy were significantly more than those before chemotherapy（P < 0.05）.（2）The syndrome of accumulation of blood stasis and toxin and syndrome of obstruction of dampness and heat after chemotherapy were significantly lower than those before chemotherapy（P < 0.05）.Conclusion:1.Before chemotherapy,the TCM syndromes of patients with advanced colon cancer were mostly positive,the disease was deficiency in excess,and the syndrome types were damp-heat syndrome,blood stasis syndrome,spleen and kidney yang deficiency syndrome,and deficiency syndrome increased significantly after chemotherapy.the main syndromes were liver,spleen and kidney deficiency,qi deficiency and blood deficiency syndromes,and finally mainly manifested as spleen and kidney yang deficiency syndrome and qi and blood deficiency syndrome.2.The TCM syndrome type of advanced colon cancer is related to the course of disease,clinical stage,degree of differentiation and other factors,the longer the course of disease,the disease tends to be mixed with deficiency and excess;the later the clinical stage is,the more the syndrome of blood stasis and toxin accumulation,the more prominent the appearance of blood stasis;In the disease site,the syndrome types of colon cancer located in the left colon before chemotherapy were mainly damp-heat obstruction syndrome and blood stasis toxin accumulation syndrome,while those in the right colon were mainly deficiency syndrome after chemotherapy.the higher the degree of differentiation,the more prominent the performance of deficiency syndrome such as spleen and kidney yang deficiency syndrome,and the lower the degree of differentiation is,the disease is more inclined to the syndrome of accumulation of blood stasis and dampness-heat.3.Traditional Chinese medicine should pay attention to the influence of emotional factors on patients in the differential treatment of advanced colon cancer,and timely use drugs such as replenishing qi,invigorating qi,regulating spleen and kidney,reaching emotion and other drugs to assist tumor treatment.
